Advanced-stage high grade serous ovarian cancer (HGSC) initially responds to platinum-based chemotherapy; however, most HGSC tumors recur and develop chemotherapy-resistance, making recurrent ovarian cancer (OC) a fatal disease. Ovarian cancer stem cells (OCSCs), a subpopulation of cells capable of surviving treatment and repopulating tumors, contribute to disease relapse, and understanding the mechanisms(s) responsible for OCSC survival is critical for developing strategies to overcome acquired resistance and prevent tumor recurrence. However, whether platinum-induced OCSC enrichment results from the selection of pre-existing OCSCs or chemotherapy-induced cellular plasticity, and the molecular mechanisms underlying these processes, remain poorly understood. Here, we demonstrated that cisplatin treatment not only enriched for OCSCs but promoted the conversion of ALDH- cells to ALDH+ cells, revealing chemotherapy-induced plasticity as a potential mechanism for OCSC expansion. This conversion was associated with nuclear receptor signaling pathways, vitamin D receptor signaling (VDR/RXR) and retinoic acid receptor signaling (RAR/RXR). Importantly, activation of VDR/RXR signaling with vitamin D reduced stemness phenotypes and the cisplatin induced conversion to ALDH+ cells. In addition, vitamin D combined with cisplatin decreased tumor volume in vivo compared to either treatment alone. In contrast, cisplatin treatment increased RAR/RXR signaling, supporting an associative role for ligand-dependent nuclear receptor signaling in OCSC plasticity. Together, these findings reveal that platinum chemotherapy can actively promote the acquisition of stem-like properties and identifies VDR/RXR signaling as a potential regulatory mechanism of OCSC plasticity. Targeting this pathway with vitamin D may provide a therapeutic strategy to target OCSCs and improve responses to platinum-based chemotherapy.

Importance Elevated low-density lipoprotein cholesterol (LDL-C) is a modifiable risk factor for cardiovascular disease, the leading cause of premature death worldwide. Assessing the LDL-C–related burden is critical for guiding prevention and treatment strategies. Objectives To estimate the global, regional, and national burden of ischemic heart disease and ischemic stroke attributable to elevated LDL-C (relative to 35-54 mg/dL) from 1990 to 2023 and to quantify the contributions of population growth, aging, risk-deleted burden, and exposure changes to burden trends. Design, Setting, and Population This comparative risk assessment, part of the Global Burden of Disease Study 2023, estimated population-level LDL-C exposure and associated health loss in 204 countries and territories. Mean LDL-C levels were estimated using spatiotemporal gaussian process regression based on 806 studies across 161 countries. Relative risks were derived from meta-analyses of 38 randomized clinical trials. Population-attributable fractions for deaths and disability-adjusted life-years (DALYs) were estimated by age and sex for adults aged 25 years or older from 1990 to 2023, with 95% uncertainty intervals. Exposure Population-level LDL-C concentrations. Main Outcomes and Measures Population-attributable fractions, counts, and rates (all ages and age standardized per 100 000) of LDL-C–attributable deaths and DALYs from ischemic heart disease and ischemic stroke, with uncertainty intervals. Results In 2023, elevated LDL-C accounted for 3.6 million deaths (95% uncertainty interval, 2.2-5.4 million; 6.0% of global mortality) and 90.7 million DALYs (95% uncertainty interval, 58.9-123.3 million; 3.2% of DALYs). Although global all-ages rates remained stable, age-standardized death and DALY rates decreased by 45.6% and 39.5%, respectively, since 1990. In 2023, age-standardized LDL-C–attributable DALY rates were highest in Eastern Europe and lowest in high-income Asia-Pacific. One-third of the global LDL-C burden occurred in India and China. Population growth and aging drove the increasing burden, with notable regional disparities in LDL-C exposure and risk-deleted DALY rates shifting toward middle-sociodemographic settings. Conclusions and Relevance Despite declining age-standardized rates, the absolute LDL-C burden has increased since 1990 due to demographic changes and has shifted toward middle-sociodemographic countries. Measurement and surveillance gaps persist. Strengthened prevention, diagnosis, and treatment access strategies are essential to mitigate the health burden of LDL-C.

Most genetic variants associated with complex traits are hypothesized to regulate gene expression. To understand the genetics underlying gene expression variability, we characterized 14,324 RNA-sequencing samples from the Trans-Omics for Precision Medicine program and performed expression and splicing quantitative trait locus (e/sQTL) analyses in six tissues and cell types, including whole blood ( n = 6454) and lung ( n = 1291). We detected tens of thousands of secondary cis-e/sQTLs, showing that secondary cis-e/sQTL discovery remains unsaturated. We fine-mapped UK Biobank–derived genome-wide association study (GWAS) signals from 164 traits and identified e/sQTL colocalizations for 10,611 GWAS signals, including 7096 that colocalize with secondary e/sQTLs. Our results suggest that even larger e/sQTL analyses will uncover additional secondary e/sQTLs, further benefiting GWAS interpretation.

Abstract Rationale Pulmonary hypertension (PH) is classified into five groups. Diagnosis requires multiple tests delaying treatment. This study compared patient and diagnostic characteristics for pulmonary arterial hypertension (PAH; Group 1 PH), Group 2-5 PH, and non-PH using machine-learning in an Electronic Health Record (EHR) dataset. This is novel as prior studies use claims data which may lack hemodynamic parameters. Methods This study analyzed adult patient records from a HIPPA-complaint EHR dataset from Mayo Clinic (2015-2024). Cohort definitions: PAH: right heart catheterization (RHC) data [mPAP>20 mmHg, PVR>2 WU, PCWP≤15 mmHg]; ≥2 PAH ICD diagnosis codes or positive natural language processing (NLP) sentiments separated by ≥ 30 days; and use of at least one PAH medication within six months after index date. Group 2-5 PH: RHC (mPAP>20mmHg) and ICD codes or positive NLP sentiment separated by ≥ 30 days for corresponding PH subtype. Non-PH: mPAP ≤ 20 mmHg and/or TRV ≤ 2.8 m/s. Index was the earliest NLP or ICD for PAH, earliest RHC for group 2-5 PH and most recent TRV/ mPAP for the non-PH cohort. Clinical variables were analyzed one-year before study index acknowledging data missingness as a limitation of retrospective dataset studies. Results 593 PAH, 817 Group 2-5 PH (67% with group 2 PH), and 11,600 non-PH patients (1:10 propensity-matched) were identified. Age, race, and symptoms were similar across cohorts. PAH had the highest proportion of females (60%) [Group 2-5 PH (50%) and non-PH cohort (53%)]. Clinical characteristic comparison is presented in Table 1. Comorbidities differed across cohorts. RHC: As expected, compared to Group 2-5, the PAH cohort had higher mPAP and PVR and lower PCWP. confirming a distinctly pre-capillary profile. ECHO: Compared to Group 2-5 PH, the PAH cohort had more pronounced right ventricular (RV) pressure overload and dysfunction and more impaired RV longitudinal strain, while TAPSE was similar. At the same time, left heart structure was less affected in PAH compared to Group 2-5. Among laboratory tests, serum creatinine was higher in Group 2-5 and ANA was higher in PAH cohort. Compared to the no-PH cohort, PAH and Group 2-5 cohorts exhibited clinically non-normal RHC and ECHO findings. Conclusion Although symptoms and demographic characteristics were similar among patients with and without PAH, unique RHC, ECHO, laboratory profiles and comorbidities differentiated PAH patients. Screening techniques using these distinct profiles could be considered to facilitate earlier diagnosis and treatment of PAH in clinical settings. ABSTRACT The standard treatment for Haemophilia B (HB) involves frequent intravenous (IV) infusions of recombinant factor IX (FIX), which imposes a significant financial burden on patients. FIX delivery as messenger RNA using lipid nanoparticles (LNPs) presents a promising alternative, but achieving prolonged and effective protein expression remains challenging. To this end, we developed Galactosylated Lipid Nucleic Acid complex (GaLiNAx) to enhance the delivery of chemically modified FIX mRNAs. The nanocarrier encompasses an ionizable galactosylated lipid to enable receptor‐mediated endocytosis and a transfection‐competent cationic lipid for optimal mRNA complexation. We incorporated truncated albumin UTRs and base modifications in functional FIX mRNA to enhance hepatic translation. In an HB mouse model, GaLiNAx demonstrated therapeutically relevant FIX protein expression for about one month after single dose offering a cost‐effective option. Safety assessments indicated that multiple doses of GaLiNAx could be administered without toxicity to any major organs. These results highlight the potential of long‐lasting mRNA therapy as an effective protein replacement strategy for HB, paving the way for future mRNA therapeutics targeting other monogenic liver disorders.

Abstract Objectives Determine differences in quantitative stenosis severity measurements for ultra-high-resolution (UHR) photon-counting detector (PCD) coronary CT angiography (cCTA) relative to energy-integrating detector (EID) cCTA in a large patient cohort. Methods Adult participants seen between November 2022 and March 2023 underwent a clinical dual-source EID-CT cCTA and a research dual-source PCD-CT cCTA on the same day. Percent diameter stenosis (PDS) was measured, and stenosis severity was assigned based on the PDS of the most severe lesion per patient to determine a coronary artery disease reporting and data system (CAD-RADS) score. Agreement between EID-CT and PCD-CT for PDS and CAD-RADS was determined. Results A total of 112 participants were enrolled, yielding 82 subjects with at least 1 stenosis at PCD-CT (mean age, 68.0 ± 10.8 years; 69.5% [57/82] males). A total of 177 paired stenoses were quantified. The percent decrease in mean PDS from PCD-CT (24.3%) to EID-CT (29.4%) was 17.3% (median difference in PDS: −5.0%, 95% CI: −5.7% to −4.0%). The assigned CAD-RADS score changed in 31/82 subjects. In 11/31 subjects, the most severe stenosis was missed at EID-CT due to partial volume averaging of a small calcification, yet PCD clearly identified stenosis at those locations. In 20/31 subjects, the CAD-RADS score decreased at PCD-CT due to the decreased calcium blooming resulting from the improved spatial resolution. Conclusions UHR PCD cCTA decreases quantitative measures of stenosis severity when a stenosis is identified at EID, leading to decreases in CAD-RADS assignments. Advances in knowledge With its improved spatial resolution, PCD-CT identifies stenoses missed on EID-CT.

Abstract NF-κB signaling can be subdivided into canonical and noncanonical pathways, culminating in the transcriptional activity of RELA and RELB, respectively. However, the upstream signals that activate these transcription factors and their specific regulatory roles in pancreatic ductal adenocarcinoma (PDAC) remain incompletely understood. Here, we demonstrate that TNFα is the primary activator of canonical NF-κB signaling via RELA, while TWEAK engages noncanonical signaling through RELB in PDAC. Using transcriptome-wide gene expression, genome-wide occupancy profiling, and epigenome mapping, we delineate distinct temporal dynamics and regulatory activities of each pathway. Single-cell RNA-seq analysis and multiplex immunofluorescence staining in primary PDAC samples further reveal extensive and distinct interactions with components of the tumor microenvironment. Motif analysis of RELA- and RELB-bound regions uncovers a particularly strong association of RELB with AP1 elements. Notably, while RELA binds to both open and closed chromatin, RELB exclusively occupies regions with pre-existing chromatin accessibility and AP1 co-binding. Collectively, these findings underscore the distinct and complementary roles of TNFα and TWEAK in PDAC, with TNFα engaging a broader transcriptional program via RELA and TWEAK selectively targeting genes marked by accessible chromatin and epigenetic activity. This study provides critical insights into the regulatory dynamics of NF-κB signaling in PDAC, highlighting unique and complementary functions of RELA and RELB in modulating downstream gene expression. Significance TNFα and TWEAK selectively activate RELA and RELB in pancreatic cancer, revealing that RELB-driven noncanonical signaling depends on chromatin accessibility and AP-1, uncovering distinct, targetable epigenetic dependencies from canonical signaling. Abstract Photon-counting detector CT (PCD-CT) represents a significant advancement in medical imaging, particularly for musculoskeletal (MSK) applications. Its primary innovation lies in enhanced spatial resolution, which facilitates improved detection of small anatomical structures such as trabecular bone, osteophytes, and subchondral cysts. PCD-CT enables high-quality imaging with reduced radiation doses, making it especially beneficial for populations requiring frequent imaging, such as paediatric patients and individuals with multiple myeloma. Additionally, PCD-CT supports advanced applications like bone quality assessment, which correlates well with gold-standard tests, and can aid in diagnosing osteoporosis and assessing fracture risk. Techniques such as spectral shaping and virtual monoenergetic imaging further optimize the technology, minimizing artefacts and enhancing material decomposition. These capabilities extend to conditions like gout and haematologic malignancies, offering improved detection and assessment. The integration of artificial intelligence could enhance PCD-CT’s performance by reducing image noise and improving quantitative assessments. Ultimately, PCD-CT’s superior resolution, reduced dose protocols, and multi-energy imaging capabilities will likely have a transformative impact on MSK imaging, improving diagnostic accuracy, patient care, and clinical outcomes.

Abstract Quantitative computed tomography (CT) enables the measurement of biophysical processes characterized by morphology, composition, flow, and/or motion to aid in clinical diagnosis and intervention. Since its initial application for determining bone mineral density for skeletal fragility assessment, quantitative CT has continued to evolve alongside CT’s technological advancements. A key challenge facing quantitative CT is the lack of standardization pertinent to image acquisition, reconstruction, and image analysis. With the introduction of spectral CT involving dual-energy approaches and photon-counting detectors (PCD), we are now able to obtain detailed information regarding mass densities of endogenous and exogeneous materials. Furthermore, energy-resolved CT yields spectral image types (eg, virtual monoenergetic image) that are, in principle, independent of tube potential and patient size. This paves the way for workflow standardization and helps improve the consistency and reproducibility of CT-derived measurements. In this article, we review clinical applications of quantitative CT, discuss key challenges associated with quantitative CT and its adoption into routine practice, and outline the unique benefits ushered by new CT technologies such as PCD-CT towards improving quantitative imaging.

Abstract Objectives To assess quantitative and qualitative image characteristics in phantoms and patients using a simultaneous ultra-high resolution (UHR) and multi-energy (ME) dual-source cardiac scan mode in a photon counting detector (PCD) CT scanner. Methods UHR-ME cardiac scans were performed to determine the spatial resolution and multi-energy performance. UHR reconstructions were completed to measure transverse and axial spatial resolution in terms of modulation transfer function (MTF) and slice sensitivity profile (SSP). Virtual monoenergetic images (VMIs), virtual non-contrast (VNC) images, and iodine concentration images were created using moderately sharp reconstruction kernels to assess ME performance. With IRB approval, coronary CT angiograms were acquired in 19 patients using the UHR-ME mode, from which iodine contrast-to-noise ratio (CNR) was measured. Various ME post-processing tasks were performed to show the utility of the spectral capabilities. Results VMIs showed a mean CT number error of 6.0%, while iodine accuracy was 0.71 mg/cc (root-mean-square-error). Spatial resolution matched well with a standard UHR mode. Patient images showed a trend of higher CNR at lower keV, while higher keV images showed decreased calcium blooming. ME techniques to separate iodine and calcium were used successfully. Conclusions The combination of UHR-ME cardiac mode retains UHR image quality while providing flexibility for ME post-processing. Advances in knowledge Cardiac UHR-ME PCD-CT scanning offers simultaneous UHR and ME imaging without loss of spatial or temporal resolution or quantitative accuracy.
