Importance The choice of induction agent during rapid sequence intubation (RSI) of critically ill adults may affect clinical outcomes. Although ketamine and etomidate are frequently used for RSI, their comparative effectiveness remains controversial. Objective To compare the safety of etomidate vs ketamine for emergency RSI in critically ill adults. Design, Setting, and Participants This cohort study used a target trial emulation with observational data collected between March 1, 2022, and April 30, 2024, from 18 emergency departments across Brazil, with data from the Brazilian Airway Registry Cooperation. Adults who underwent RSI and received either etomidate or ketamine as a sole hypnotic agent were included; those with preintubation cardiac arrest or immediate postintubation transfer were excluded. Exposure Administration of etomidate or ketamine as the induction agent for RSI. Main Outcomes and Measures The primary outcome was 28-day in-hospital mortality. Secondary outcomes included 7-day in-hospital mortality, first-attempt intubation success, and major adverse events (new hemodynamic instability, severe hypoxemia, and cardiac arrest) within 30 minutes after intubation. Inverse probability of treatment weighting was used to adjust for confounding. Risk ratios (RRs) and risk differences (RDs) with 95% CIs were used to compare outcomes between groups. Results Among 1810 patients (median age, 64 years [IQR, 50-74 years]; 1048 men [57.9%]), 514 received ketamine and 1296 received etomidate. The median shock index was higher in the ketamine group than the etomidate group (0.81 [IQR, 0.65-1.01] vs 0.76 [IQR, 0.59-0.99]), and preintubation vasopressor use was more common in the ketamine group than the etomidate group (191 of 514 [37.2%] vs 391 of 1296 [30.2%]). Weighted 28-day mortality was higher with etomidate than ketamine (60.5% [95% CI, 57.2%-63.8%] vs 54.4% [95% CI, 45.0%-63.9%]; RR, 1.14 [95% CI, 1.03-1.27]; RD, 7.6% [95% CI, 2.0%-13.3%]). Seven-day mortality was also higher with etomidate than ketamine (35.2% [95% CI, 32.0%-38.3%] vs 30.1% [95% CI, 23.5%-36.7%]; RR, 1.19 [95% CI, 1.04-1.35]). New hemodynamic instability within 30 minutes after intubation was more frequent with ketamine (24.2% [95% CI, 20.4%-28.0%] vs 18.9% [95% CI, 16.7%-21.0%]; RR, 0.78 [0.64-0.95]). There were no statistically significant differences in the other secondary outcomes. Conclusions and Relevance In this cohort study of critically ill adults undergoing RSI, etomidate use was associated with higher in-hospital mortality at 7 and 28 days compared with ketamine. These findings highlight the need for definitive randomized clinical trials to compare both agents.

Abstract Objectives Patient education is increasingly acknowledged as an important aspect of the management of systemic lupus erythematosus (SLE). The aim of the study was to develop the SLE Knowledge Assessment score (SLAKE), a digital multilingual self-assessment tool designed to quantify essential SLE knowledge. Methods International healthcare professionals (HCPs) and patient representatives engaged in a multi-step process to: identify essential SLE knowledge domains, select key domains via rating, and generate an item bank of 394 questions across 11 domains, which was then adapted into 19 languages. For validation, participants completed 44 questions (including 33 randomly selected), with scores calculated for total knowledge and the 11 specific domains. Statistical analyses examined associations between scores and demographic, clinical, and educational variables. Results SLAKE was used by 1182 SLE participants (1120 [94.8%] women, median age: 45 years [IQR: 35–54 years]), with a median SLE duration of 10 years (IQR: 4–20 years). The median SLAKE score was 37 (IQR: 34–40) of a maximum of 44 points while the median score across the 11 SLAKE domains ranged between 3 and 4 over a maximum of 4 points. There was a significant positive association between SLAKE score and SLE duration (p= 0.006), previous participation to a patient education course or a patient training for lupus (p< 0.0001) and the education level (p< 0.0001) but not with age (p= 0.48) or gender (p= 0.39). Conclusion SLAKE is a valid, multilingual, digital self-assessment tool that effectively measures essential SLE knowledge. Its randomized question bank and domain-specific scoring enable targeted education, ultimately supporting better disease management.

Background: Body contouring is an increasingly common trend in aesthetic plastic surgery. In the pursuit of ideal body proportions, anterior thoracic cage deformities, especially lower rib cartilage prominence have emerged as a frequent source of dissatisfaction. To address this concern, the anterior aesthetic rib cage remodeling with osteosynthesis (ARCO) technique was developed as an innovative method to reshape the anterior rib cage. Methods: A retrospective study was conducted in Colombia on patients who underwent the ARCO procedure. Preoperative and postoperative rib cage perimeter measurements were recorded. Spirometry was performed preoperatively and during long-term follow-up to assess potential changes in respiratory volumes. Intraoperative peak pressure and pulmonary compliance values were monitored to evaluate procedural safety. Postoperative satisfaction was assessed using the BODY-Q questionnaire, and all complications were documented. Results: A total of 34 patients were included, with a mean age of 34 years. A significant reduction in the inferior rib cage perimeter at the level of the eighth rib was observed (Mann-Whitney U =190, P < 0.001), with a mean decrease of 2 cm. Intraoperative peak inspiratory pressure and pulmonary compliance remained stable across all procedural stages. Spirometric analyses revealed no restrictive changes in respiratory volumes. Conclusions: The ARCO technique offers an effective approach for reducing chest circumference and enhancing the aesthetic contour of the anterior rib margin while achieving high patient satisfaction. It involves a controlled greenstick fracture followed by osteosynthesis to create a stable and natural rib curvature, reducing the risk of malunion and eliminating the need for postoperative corsets.

Abstract Background Partial breast irradiation (PBI) is an effective treatment for early-stage breast cancer. However, evidence comparing different PBI modalities is limited. Methods This trial emulation uses electronic health records from a multi-state large tertiary health system. Three PBI modalities were compared: proton radiation therapy, photon radiation therapy, and applicator-based brachytherapy. Inverse probability weighting (IPW) was used to balance confounders. The primary outcome was ipsilateral breast recurrence (IBR). Results Between 2013 and 2023, 1041 women with early-stage breast cancer were treated with PBI. Of these, 1026 patients were included in the analyses. The median age was 66 years; 22.51% had ductal carcinoma in situ (DCIS), and 97.56% were estrogen receptor positive. With median follow-up of 38.2 months, the rate of IBR in the overall group was 3.05% (95% confidence interval [CI] = 1.99% to 4.65%) at 3 years. After IPW, compared with photon PBI, the hazard ratio for IBR for proton PBI was 2.11 (95% CI = 0.70 to 6.33) and for brachytherapy, 3.86 (95% CI = 1.39 to 10.69). Compared with proton PBI, the hazard ratio for IBR for brachytherapy was 1.83 (95% CI = 0.88 to 3.81). IBR risk was similar across PBI modalities for patients with tumor size ≤ 10 mm and in patients treated for DCIS. Among patients with tumor size >10 mm, the hazard ratio for IBR for brachytherapy was 7.64 (95% CI = 1.64 to 35.58) and 3.59 (95% CI = 1.22 to 10.56) relative to photons and protons, respectively. Conclusion PBI with applicator-based brachytherapy was associated with higher IBR than photon or proton PBI in patients with tumor size > 10 mm, suggesting the need for more careful PBI patient selection for this modality.

Abstract Introduction Bruton tyrosine kinase inhibitors (BTKis) have changed the treatment algorithm for patients with high-risk relapsed/refractory (R/R) chronic lymphocytic leukemia (CLL). Efficacy was demonstrated for zanubrutinib and ibrutinib in high-risk R/R CLL patients in the ALPINE (NCT03734016) trial, for ibrutinib and acalabrutinib in the ELEVATE-RR (NCT02477696) trial, and for acalabrutinib in the ASCEND (NCT00135226) trial. While there is limited head-to-head comparative trial data of all covalent BTKis in the treatment of high-risk R/R CLL, a previously published network meta-analysis (NMA) used data from the ALPINE, ELEVATE-RR, and ASCEND trials, and reported that zanubrutinib demonstrated significantly improved relative efficacy compared to ibrutinib and acalabrutinib in high-risk R/R CLL (Shadman 2025). Within this NMA, high-risk R/R CLL populations were defined based on the prespecified definitions within each trial, including patients with del(17p) and/or TP53 mutations in the ALPINE and ASCEND trials, and del(17p)/del(11q) in the ELEVATE-RR trial. This study aimed to compare zanubrutinib versus ibrutinib and acalabrutinib in high-risk R/R CLL by calculating the number needed to treat (NNT) to avoid one progression or death. Methods A model was developed to evaluate the NNT among high-risk R/R CLL patients to avoid a progression or death. Progression free survival (PFS) data for zanubrutinib in high-risk patients were extracted from the ALPINE trial. PFS values for ibrutinib and acalabrutinib were derived from the previously published NMA study. PFS for ibrutinib and acalabrutinib were calculated by applying PFS hazard ratio from the NMA results to the zanubrutinib PFS value for the high-risk population in the ALPINE trial. 24-month PFS (72.6% for zanubrutinib, 52.0% for ibrutinib, and 55.9% for acalabrutinib) were used for the base case analysis of the model. Sensitivity analyses were conducted to examine the impact of alternative PFS inputs for ibrutinib, derived directly from the high-risk populations in ALPINE trial (unadjusted). Results The base case results from the NNT model indicate that for the treatment of patients with high-risk R/R CLL over 24-months, every five patients treated with zanubrutinib instead of ibrutinib results in the avoidance of one disease progression or death and every six patients treated with zanubrutinib instead of acalabrutinib results in the avoidance of one disease progression or death. Applying the model result to a hypothetical scenario of 100 patients with high-risk R/R CLL treated with zanubrutinib versus ibrutinib for two years finds approximately 20 patients will avoid disease progression events or deaths. Treating the same population with zanubrutinib versus acalabrutinib for two years will result in the avoidance of 17 disease progression events or death. Sensitivity analysis showed consistent NNT results. Conclusions The NNT analysis suggests that among high-risk R/R CLL patients, zanubrutinib is associated with more favorable clinical outcomes compared to ibrutinib and acalabrutinib. Over 24 months, zanubrutinib demonstrated a NNT of five to prevent one disease progression or death compared to ibrutinib, and an NNT of six compared to acalabrutinib, in high-risk R/R CLL patients. Study findings should be interpreted based on modeling assumptions and potential patient population differences across trials.

Abstract Background The treatment (tx) landscape for chronic lymphocytic leukemia (CLL) has evolved, with National Comprehensive Cancer Network (NCCN) guideline-preferred novel therapies (NTs), such as acalabrutinib, zanubrutinib, and venetoclax-based regimens, now preferred over ibrutinib and chemoimmunotherapy (CIT) in front-line (1L) settings. We previously showed that patients (pts) from different racial/ethnic groups may not have equitable access to NTs. Here, we examined racial/ethnic inequities in 1L NT utilization among US pts with CLL and potential social determinants of health (SDOH) drivers of these inequities. Methods This retrospective cohort study utilized the US-based, electronic health record-derived de-identified Flatiron Health Research Database, linked to neighborhood (US Census track or block group) data from American Community Survey and Agency for Healthcare Research and Quality datasets. Eligible pts included adults with CLL who started 1L tx 01/01/2019-07/31/2024. The primary outcome was receipt of 1L therapy (CIT vs preferred NTs, ibrutinib vs preferred NTs), with NCCN guideline-preferred NTs (v2.2023-v3.2025) as a reference. Associations between race/ethnicity (White, Black, Hispanic) and 1L tx types were assessed using logistic regressions, adjusting for age, sex, year of 1L start, immunoglobulin heavy chain variable region status, and del17p/TP53 status. To better understand which SDOH factors contribute to racial and ethnic inequities in tx utilization, we performed mediation analysis using Multiple Mediation Analysis implemented through nonlinear multiple additive regression tree models. Individual-level (Eastern Cooperative Oncology Group status, practice type, insurance) and 20 area-level SDOH factors measuring social deprivation (defined as limited access to economic, social, neighborhood, physical, or healthcare resources) were assessed as potential mediators. Results Of the 4452 pts included in the study, 3717 were White (83%), 371 were Black (8%), 209 were Hispanic (5%), and 155 were Other race (4%). Black and Hispanic pts were more likely to live in neighborhoods with higher social deprivation, such as residential segregation (predominant race/ethnicity in the area: White, Black, Hispanic, Diverse), with no internet access, no vehicle ownership, and no health insurance coverage. Overall, 37% of pts received a preferred NT, highest among White (38%) pts, followed by Black (35%) and Hispanic (26%) pts. Compared with White pts, Hispanic pts were more likely to receive CIT than preferred NTs (adjusted odds ratio [aOR] 2.12; 95% confidence interval [CI] 1.46-3.09); Black (aOR 1.44; 95% CI 1.05-1.97) and Hispanic pts (aOR 1.83; 95% CI 1.17-2.85) were more likely to receive ibrutinib than preferred NTs. Pts living in areas with the highest social deprivation were less likely than their counterparts in lowest social deprivation neighborhoods to receive preferred NTs. Specifically, residence in predominantly Black or Hispanic neighborhoods (26% or 27% vs 39% predominantly White), areas with lowest socioeconomic status (32% vs 40%), and neighborhoods with the lowest levels of internet access (32% vs 39%), vehicle ownership (32% vs 39%), and health insurance coverage (34% vs 41%) was associated with lowest receipt of preferred NTs. In the mediation analysis, individual- and area-level SDOH factors explained 94.4% (95% CI 2.9-185.9) of the observed Black-White inequity in receipt of ibrutinib versus preferred NTs; residential segregation appeared to be the most important mediator (59.7%; 95% CI −3.5 to 123.0). Similarly, the Hispanic-White inequity in receipt of preferred NTs was driven by the combined effect of area-level SDOH and individual-level variables (88.5%; 95% CI 45.6-131.3), with residential segregation as the main driver (48.3%; 95% CI 10.7-85.9), followed by practice setting (9.8%; 95% CI −3.5 to 23.1). In contrast, measured SDOH factors did not explain the inequity in receipt of CIT versus preferred NTs between Hispanic and White pts (21.5%; 95% CI −8.3 to 51.4), suggesting that other factors may contribute to the observed inequities. Conclusion Black and Hispanic pts with CLL were less likely than White pts to receive preferred NTs. A significant proportion of these disparities was explained by area-level SDOH, particularly residential segregation. These findings underscore the need to address structural barriers to ensure equitable access to emerging, guideline-recommended treatments.

Abstract Background Chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) is the most common leukemia in US adults and incidence increases with age. Although the treatment (tx) landscape has evolved, it is unclear whether tx selection differs for older patients (pts), given concerns around comorbidities, toxicity, and efficacy. There is a need to better understand pt characteristics, tx patterns, and biomarker use among pts aged ≥65 yrs with CLL/SLL in the current therapeutic era. Methods A retrospective longitudinal study was conducted using the de-identified 100% Medicare Fee-for-Service (FFS) Research Identifiable Files. Included pts were diagnosed with CLL/SLL at age ≥65 and initiated first-line (1L) tx 1/1/2020-9/30/2024 (with ≥12 mo in database prior to 1L tx start). Index date was defined as date of 1L tx start, and pts were followed to death, end of Medicare enrollment, or study end, whichever occurred first. 1L tx was defined as any CLL pharmacologic tx within the first 90 days of index date. Tx utilization was summarized as the number and proportion of pts who received each tx at each line. Descriptive statistics were used to summarize demographic and social characteristics and baseline comorbidities, in all pts and by 1L tx. Proportion and timing of documented biomarker orders any time prior to index were summarized by test type (CLL fluorescence in situ hybridization [FISH], TP53 DNA sequencing and immunoglobulin heavy chain variable region [IGHV] test) and by 1L tx. Results Of the 21,008 pts included, median age was 76.4 yrs at 1L tx, with 43% of pts aged 65-74 yrs, 43% aged 75-84 and 14% aged ≥85. The majority of pts were male (58%) and White (91%). About 8% and 9% of pts had dual eligibility status and low-income subsidy, respectively. Median Charlson Comorbidity Index (CCI) was 4 (range: 1-21), with 40% of pts having CCI ≥5. Covalent Bruton tyrosine kinase inhibitor (cBTKi) monotherapies (mono) were most common at 1L tx (41%), including 10% zanubrutinib (zanu), 15% ibrutinib (ibr), and 16% acalabrutinib (acala). B-cell lymphoma 2 (BCL2)+anti-CD20 monoclonal antibody (mAb) was less common (11%), and chemoimmunotherapy (CIT) was used by 13% of pts, mostly BR (8%) and FCR (3%). Other less common tx included cBTKi+anti-CD20mAb (6%) or cBTKi+BCL2 therapies (1%). Among remaining pts, most received anti-CD20mAb (22%). The use of cBTKi mono increased from 41.7% in 2020 (ibr 30.6%; acala 11.1%) to 47.6% in 2024, driven by increased use of zanu (27.8%; acala 15.7%; ibr 4.2%). BCL2+anti-CD20mAb increased from 9.2% in 2020 to 12.6% in 2024. Pts treated with cBTKi mono were older than those with BCL2+anti-CD20mAb (median age 77.2 vs 74.5), and more likely to have dual eligibility status (10% vs 4%) or low-income subsidy (11% vs 5%). More pts who received zanu mono had CCI ≥5 (39%) versus acala (36%), ibr (35%), or BCL2+anti-CD20mAb (32%). Pts with BCL2+anti-CD20mAb were less likely to have diabetes (23% vs 26%), congestive heart failure (CHF; 16% vs 19%), chronic pulmonary disease (23% vs 27%), and renal diseases (23% vs 27%). Pts with ibr were less likely to have myocardial infarction (29% vs 34%), coronary artery disease (22% vs 25%), CHF (16% vs 19%), and atrial fibrillation (12% vs 18%). Overall, 69% of pts had FISH testing prior to 1L tx, but test orders for TP53 (12%) or IGHV (28%) status were infrequent. Pts with ibr had the lowest rates of TP53 tests (6%), whereas pts with CIT had the lowest rates of IGHV tests (16%). The timing of tests showed a binomial distribution where most pts received FISH, TP53, or IGHV tests either ≤60 days prior to 1L tx (46%, 50%, 33%), or >1 yr before 1L tx (30%, 22%, 43%). Among pts who received ≥2 lines of therapy for CLL (n=4428; 21%), cBTKi mono was the most common in 2L (41%), including 14% zanu, 17% acala, and 10% ibr. CIT and BCL2+anti-CD20mAb were used in 12% and 9% of pts, respectively. Only 634 (3%) pts had 3L+ tx, of whom 33% received cBTKi mono, including 16% zanu, 12% acala, and 5% ibr (5%). Conclusion This large real-world study found that cBTKi mono was the most common 1L tx choice among pts aged ≥65 with CLL/SLL, with increased use of next-generation cBTKi. Pts with more baseline comorbidities were less likely to receive ibr or BCL2-based tx. FISH testing was not performed in nearly one-third of pts, and most pts did not have TP53 or IGVH sequencing prior to tx. Future studies are needed to continue to evaluate evolving tx paradigms in older pts with CLL/SLL.

Abstract Myeloid neoplasia encompasses a heterogeneous and complex molecular landscape whereby the distinct combinatorial patterns associated with clinical phenotypes are difficult to delineate. Given that genetic heterogeneity can be a consequence of the topology of the fitness landscape (peaked/non-peaked=high vs low fitness), we investigated co-mutational patterns in TP53 context as an example of hallmark genomic heterogeneity. Indeed, TP53 gene is characterized by hypomorphic/loss-of-function lesions, truncating mutations (frameshifts, stop codons), missense mutations often having a dominant-negative effect and loss of heterozygosity by chromosome 17p deletion driving clonal expansion. Clinical phenotypes of TP53 mutants are also heterogeneous. Here, we stratify TP53 mutations based on our previous published data1 whereby patients with single-hit TP53 and variant allelic frequency (VAF) >50% were classified as obligatory biallelic, patients with VAF between 23-50% as probable biallelic and <23% as probable monoallelic. A threshold of VAF 23% was optimal to separate monoallelic vs biallelic and was based on all-cause mortality. A tree-based model (RandomForestSRC) was used to build a survival model with VAF as a single covariate and minimum node size set to 15 to optimize the cut-off. Using this cut-off, we delineate the epistatic landscape and potential interactions among different TP53 mutational spectra and its fitness. We curated a cohort of 800 patients with TP53 mutations from our institution. In total 36% of the cases carried single hits while 64% had double hits genomic configuration. About 70% of the mutations were missense and were detected at canonical sites (R175H, Y220C, M237I, R273H, R282W, R248Q). We took into consideration 35 features and clustered our cohort of TP53 mutants using factor analysis. When we stratified the cohort based on our predetermined VAF threshold, we found that majority of the variants with high VAF, to be deemed as obligatory biallelic, co-occurred with complex karyotype. These cases were very homogeneous hence had on average a low number of additional variants, likely due to TP53 clones having reduced fitness in the presence of additional mutations. Conversely, patients without complex karyotype were mostly assigned to the monoallelic group due to low to median VAFs. Consequently, these samples had on average a higher number of additional hits, hence a heterogeneous molecular landscape. To substantiate this finding, we noticed that among monoallelic patients two characteristic pairs were observed (TP53-TET2 and TP53-SF3B1) and showed different patterns according to karyotypic associations. Indeed, both pairs in normal karyotype (TP53-TET2, 65%; TP53-SF3B1, 23%) had the highest number of additional mutations on average compared to other monoallelic hits in complex karyotype suggesting that clones resulting by these pairs were advantageous and produced higher fitness driving heterogeneity when together with other mutations. As a preliminary analysis to characterize the fitness landscape of canonical missense TP53 mutations, we investigated whether distinct TP53 mutations conferred differential clonal fitness by implementing a previously published model of stochastic hematopoietic stem cell growth based on the distribution of allelic frequencies across different age groups. Given a fixed stem cell population pool and a constant mutation rate, the distribution of allelic frequencies in the population of corresponding mutations across age can inform the fitness/selection coefficient. The model predicts that high fit mutations will expand more in the population (high VAF) depending on the time the mutation is allowed to expand (age). Based on the literature, we set the stem cell pool to 100,000 and the mutation rate to 1.4E-8.2We limited our analysis to mutations found in at least 10 individual in our cohort and to single hits due to potential epistatic interactions across multiple mutations in the same gene. Given this criterion two mutations R273H and R248Q were analyzed. Interestingly, we observed a 15% increase in fitness gain for R273H compared to R248Q. In summary, mutational heterogeneity and clonal fitness might provide clues to the diversity of TP53 mutational configurations. Our model will be further optimized for the upcoming ASH meeting by incorporating temporal sampling and increasing sample size to define differences in fitness according to mutation site.

Abstract Background Watchful waiting is a common option for pts with asymptomatic FL/MZL. Anti-tumor immunization may delay or even avoid subsequent need for more toxic therapies. EO is designed from non-self-protein sequences derived from gut bacteria, including 4 HLA-A2 CD8 T cell epitopes (synthetic mimic peptides), exhibiting molecular mimicry with specific epitopes on B cell markers (CD20, CD22, CD37, BAFF-receptor). EO also contains a CD4 helper epitope UCP2. EO expands pre-existing memory CD8 T cells recognizing non-self-epitopes from gut bacteria that cross-react with B cell antigens on tumor cells. Methods Cohort 2 (C2; planned accrual 25 pts) of EONHL1-20/SIDNEY includes HLA-A2+ pts with previously untreated FL/MZL not in need of treatment. Pts receive EO (300μg/peptide) SC with adjuvant Montanide, q2 weeks (w) x 4, then q4w, for 12 total doses. For antigen-specific immune responses, peripheral blood mononuclear cells were assayed by flow cytometry using EO-mimic or B cell peptide specific tetramers without in vitro stimulation. IFN-γ ELISPOT was used to evaluate response to UCP2 and anti-CD3. Primary endpoint is objective response rate (ORR) per Lugano 2014. A pre-defined futility boundary was applied (ORR uninteresting=5%/promising=20%). Finding at least 3 of 25 pts with ORR would meet criteria to continue development (chance of 13% and 90% for true ORR of 5% and 20%, respectively). Results As of July 2025, 22 pts had started EO. Median age 59 years (range 32-86); 9 female; 19 ECOG 0/3 ECOG 1; 19 FL/3 extranodal MZL (ENMZL); 3 Ann Arbor stage I-II/8 stage III/11 stage IV; FLIPI low risk 8/intermediate 8/high 6; FLIPI-2 low risk 12/intermediate 7/high 3; 19 GELF neg/3 pos. At a median follow-up for progression of 9.4 months, 9 pts completed EO, 4 stopped for progressive disease (PD), 2 due to PI/pt decision; 7 ongoing. Median treatment duration 38 w (range 4-39). Among 19 evaluable pts, best Lugano response was 3 complete (CR) and 6 partial responses (PR) (ORR 47% [95% CI 24%–71%]; CR-rate 16% [3%-40%]), 6 stable disease (SD), and 4 PD. ORR by PET-CT was 29% (n=14) w6, 39% (n=18) w19, 46% (n=13) w42; corresponding CR rates were 0%, 11%, 15%. Median time to response 17.1 w (range 5.3-41.1). Currently there are only 2 confirmed PD events in pts with prior PR/CR. Among 8 pts with an initial indeterminate response per LyRIC 2016 followed by a confirmatory scan, 4 pts did not have confirmed PD; a possible atypical response pattern rate of 50% on EO (occurring 5.3-41.9 w after start of EO). In 16 immune response evaluable pts, 88% and 81% had expansion of CD8 T cells specific for EO mimic and B cell target peptides, respectively. Expanded specific CD8 T cells had a memory phenotype dominated by effector memory (TEM) and central memory cells. At w5, 7 clinical responders and 7 non-responders were evaluable; more responders had a positive immune response against EO mimic (Chi-square p=0.025) and B cell target (p=0.047) peptides. Max expansion (response sum for 4 EO mimic or 4 B cell target peptides at a single timepoint w3-w8) of specific CD8 T cells (% of all peripheral CD8 T cells) was higher for responders than non-responders both for EO mimic (Mann Whitney p=0.018) and B cell target (p=0.017) peptides. Pts (4) with an initial indeterminate response (LyRIC 2016) had a higher max expansion of B cell target peptide specific CD8 T cells vs 4 pts with a typical progression pattern (Mann Whitney p=0.018) and vs all other 12 pts evaluable for immune response (p=0.029). There was no difference in CD4 T cell response to UCP2 in responders vs non-responders (induction or max immunity w3-w8). Anti-CD3 expansion of T cells indicating T cell intrinsic activation potential did not correlate with clinical response (baseline or max). The most common EO-related adverse events (AEs) were Gr (Grade) 1 (10 pts) and Gr 2 (6 pts) local administration site reactions. Other related AEs in >1 pt were Gr 1 headache (3 pts) and Gr 1-2 fatigue (3 pts). Asthenia lasting 2 days was the only related Gr 3 AE. There were 4 unrelated Gr 3 AEs (gastroenteritis, GI-hemorrhage, inguinal hernia, and syncope). There were no Gr 4-5 AE. ConclusionsEO has a favorable safety profile and is preliminary associated with an ORR of 47%, exceeding the pre-specified boundary for promising activity. The speed and level of expansion of both EO mimic and B cell target peptide specific CD8 memory T cells are associated with objective response. Results will be updated.
