Plasma glucose 2H‐enrichment in positions 5 (2H5) and 2 (2H2) from deuterated water (2H2O) provides a measure of the gluconeogenic contribution to endogenous glucose production. Urinary glucuronide analysis can circumvent blood sampling but it is not known if glucuronide and glucose enrichments are equal. Thirteen subjects with impaired fasting glucose/impaired glucose tolerance and 11 subjects with normal fasting glucose and normal glucose tolerance ingested 2H2O to ∼0.5% body water and acetaminophen. Glucose and glucuronide 2H5 and 2H2 were measured by 2H NMR spectroscopy of monoacetone glucose. For normal fasting glucose/normal glucose tolerance, 2H5 was 0.23 ± 0.02% and 0.25 ± 0.02% for glucose and glucuronide, respectively, whereas 2H2 was 0.47 ± 0.01% and 0.49 ± 0.02%, respectively. For impaired fasting glucose/impaired glucose tolerance, 2H5 was 0.22 ± 0.01% and 0.26 ± 0.02% for glucose and glucuronide, respectively, whereas 2H2 was 0.46 ± 0.01% and 0.49 ± 0.02%, respectively. The gluconeogenic contribution to endogenous glucose production measured from glucose and glucuronide were identical for both normal fasting glucose/normal glucose tolerance (48 ± 4 vs. 51 ± 3%) and impaired fasting glucose/impaired glucose tolerance (48 ± 2 vs. 53 ± 3%). The cardiovascular diseases such as atherosclerosis, coronary artery disease, hypertension and diastolic dysfunction have been associated with arterial stiffening and decreased ventricular compliance. Noninvasive techniques capable of quantifying elasticity and viscosity of cardiovascular tissues could facilitate early diagnosis and improve treatment. Here, we present a technique that uses focused ultrasound radiation force to excite mechanical waves in the tissue of interest and pulse echo to track the tissue deformation. Analysis of tissue deformation as a function of time using Fourier transforms allows calculation of the phase wave velocity dispersion (change of velocity as a function of frequency) of various modes of deformation. Continuum mechanics equations governing the motion of a viscoelastic plate and a tube are used to model the myocardial wall and arteries, respectively. Dispersion equations are derived for the two geometries and fit to the measured velocity dispersion to estimate tissue elasticity and viscosity. ECG-gated in vivo measurements of porcine myocardial and arterial elasticity and viscosity through a heart cycle are reported. The results show that both elasticity and viscosity increase during systole and decrease during diastole in the myocardium and arteries, consistent with underlying physiology.

Defects in polycystin-2, a ubiquitous transmembrane glycoprotein of unknown function, is a major cause of autosomal dominant polycystic kidney disease (ADPKD), whose manifestation entails the development of fluid-filled cysts in target organs. Here, we demonstrate that polycystin-2 is present in term human syncytiotrophoblast, where it behaves as a nonselective cation channel. Lipid bilayer reconstitution of polycystin-2-positive human syncytiotrophoblast apical membranes displayed a nonselective cation channel with multiple subconductance states, and a high perm-selectivity to Ca 2+ . This channel was inhibited by anti-polycystin-2 antibody, Ca 2+ , La 3+ , Gd 3+ , and the diuretic amiloride. Channel function by polycystin-2 was confirmed by patch-clamping experiments of polycystin-2 heterologously infected Sf9 insect cells. Further, purified insect cell-derived recombinant polycystin-2 and in vitro translated human polycystin-2 had similar ion channel activity. The polycystin-2 channel may be associated with fluid accumulation and/or ion transport regulation in target epithelia, including placenta. Dysregulation of this channel provides a mechanism for the onset and progression of ADPKD.

AbstractCellular and interpatient heterogeneity and the involvement of different stem and progenitor compartments in leukemogenesis are challenges for the identification of common pathways contributing to the initiation and maintenance of acute myeloid leukemia (AML). Here we used a strategy of parallel transcriptional analysis of phenotypic long-term hematopoietic stem cells (HSCs), short-term HSCs, and granulocyte-monocyte progenitors from individuals with high-risk (−7/7q−) AML and compared them with the corresponding cell populations from healthy controls. This analysis revealed dysregulated expression of 11 genes, including IL-1 receptor accessory protein (IL1RAP), in all leukemic stem and progenitor cell compartments. IL1RAP protein was found to be overexpressed on the surface of HSCs of AML patients, and marked cells with the −7/7q− anomaly. IL1RAP was also overexpressed on HSCs of patients with normal karyotype AML and high-risk myelodysplastic syndrome, suggesting a pervasive role in different disease subtypes. High IL1RAP expression was independently associated with poor overall survival in 3 independent cohorts of AML patients (P = 2.2 × 10−7). Knockdown of IL1RAP decreased clonogenicity and increased cell death of AML cells. Our study identified genes dysregulated in stem and progenitor cells in −7/7q− AML, and suggests that IL1RAP may be a promising therapeutic and prognostic target in AML and high-risk myelodysplastic syndrome.

Chromosomal translocations involving the MALT1 gene are hallmarks of mucosa-associated lymphoid tissue (MALT) lymphoma. To date, targeting these translocations to mouse B cells has failed to reproduce human disease. Here, we induced MALT1 expression in mouse Sca1 + Lin − hematopoietic stem/progenitor cells, which showed NF-κB activation and early lymphoid priming, being selectively skewed toward B-cell differentiation. These cells accumulated in extranodal tissues and gave rise to clonal tumors recapitulating the principal clinical, biological, and molecular genetic features of MALT lymphoma. Deletion of p53 gene accelerated tumor onset and induced transformation of MALT lymphoma to activated B-cell diffuse large-cell lymphoma (ABC-DLBCL). Treatment of MALT1-induced lymphomas with a specific inhibitor of MALT1 proteolytic activity decreased cell viability, indicating that endogenous Malt1 signaling was required for tumor cell survival. Our study shows that human-like lymphomas can be modeled in mice by targeting MALT1 expression to hematopoietic stem/progenitor cells, demonstrating the oncogenic role of MALT1 in lymphomagenesis. Furthermore, this work establishes a molecular link between MALT lymphoma and ABC-DLBCL, and provides mouse models to test MALT1 inhibitors. Finally, our results suggest that hematopoietic stem/progenitor cells may be involved in the pathogenesis of human mature B-cell lymphomas.

Diastolic dysfunction is characterized by the stiffening of the left-ventricular myocardium. An imaging technique capable of quantifying viscoelasticity of the left-ventricle would aid clinicians in evaluating diastolic function. Lamb wave dispersion ultrasound vibrometry (LDUV) is an ultrasound-based method for measuring viscoelasticity of plate-like materials by exciting Lamb waves and measuring change of wave velocity as a function of frequency (dispersion). Lamb wave dispersion equation is fit to the dispersion data to estimate mechanical properties. We report in vivo transthoracic studies aimed at quantifying elasticity and viscosity of a beating porcine left-ventricle using LDUV. A single ultrasound probe operating at 5 MHz was used for impulse excitation in the myocardium and to track the wave motion. A Fourier space-time analysis of the motion was used to obtain Lamb wave velocity dispersion. ECG-gated transthoracic in vivo measurements of group velocity, elasticity, and viscosity throughout a single heart cycle were obtained. Group velocity, elasticity, and viscosity in the frequency range from 50 to 500 Hz increased by at least two-fold from diastole to systole, consistent with contraction and relaxation of the myocardium.
