Objective Positive α‐synuclein seed amplification assay (SAA) is a biomarker found in most people with Parkinson's disease (PD). We explored if free‐water (FW) imaging detects microstructural differences in the brains of patients with early PD with SAA+ or SAA– status. Methods We studied patients with PD with baseline diffusion imaging and α‐synuclein SAA data from the Parkinson's Progression Markers Initiative (PPMI). We compared FW, FW corrected fractional anisotropy (FA T ), and clinical characteristics between SAA+ and SAA– groups. We also applied the Automated Imaging Differentiation for Parkinsonism (AIDP) at baseline to classify PD versus atypical parkinsonism, stratified by SAA status. Results Among 462 participants (41 SAA– and 421 SAA+), individuals with SAA+ had hyposmia and shorter motor symptom duration before baseline magnetic resonance imaging (MRI). AIDP identified PD in 92.4% (n = 427, 91.6% had SAA+) and classified 7.6% as atypical parkinsonism (n = 35, 85.7% had SAA+). At baseline, SAA+ individuals had lower FW in the superior cerebellar peduncle, compared to SAA– (pFDR < 0.05). No significant differences in FA T were found between groups. Interpretation Positive α‐synuclein SAA was associated with focal microstructural differences but did not distinguish broader diffusion MRI (dMRI) changes across FW and FA T metrics. These findings indicate that molecular confirmation of synuclein aggregation (via SAA) provides limited stratification of neurodegeneration detected by FW imaging in early PD. Abstract The genetic contributions of the X chromosome to Alzheimer’s disease (AD) remain poorly understood yet are expected to importantly shape sex differences in AD. We therefore performed large-scale X-chromosome-wide association studies (N=1,240,451), evaluating differential risk due to sex, APOE *4, and escape from X-chromosome inactivation, finding most X-linked loci appear relevant to female-biased AD etiology. In evaluating genetic pleiotropy with hormonal, lipid, and brain imaging traits, we discovered X-linked AD loci converged on white matter traits, particularly in the anterior corona radiata and splenium of the corpus callosum. Through brain-centric functional genomics analyses, we then nominated candidate causal genes, including 5 that appeared highly robust. Notably, we found the escape gene RBBP7 decreases AD risk in APOE *4 carriers likely through higher expression in excitatory neurons to counter tau-related neurodegeneration. Altogether, we provide an atlas of sex and APOE *4-informed candidate X-linked AD risk loci, genes, and mechanisms that will guide future studies.

Abstract Background Osteoarthritis is a leading cause of disability worldwide, and cell‐based treatments including adipose‐derived and bone marrow aspirate have been sought by the lay and medical community as treatment options. Objective To perform a scoping review of published literature on cell‐based injections allowed by the U.S. Food and Drug Administration for the treatment of osteoarthritis to synthesize existing evidence and identify research gaps for future evaluation. Methods A comprehensive search of five databases was executed from inception through January 2, 2025. Studies that met the inclusion criteria were original research studies written in English on Food and Drug Administration‐allowed cell‐based treatments in adults with osteoarthritis of any joint. Results The database search yielded 4257 unique records. After screening, 84 studies met the inclusion criteria, encompassing 9996 patients and a total of 10,508 procedures. The primary research study designs were cohort studies ( n = 62), focused on treatment of knee osteoarthritis ( n = 63), and described bone marrow aspirate ( n = 42) and adipose‐derived ( n = 42) treatments. Postprocedure monitoring ranged from 6 weeks to 5 years, with most studies ≤1 year ( n = 59). Patient‐reported outcomes were reported in 83 of 84 studies; few provided imaging outcomes including magnetic resonance imaging ( n = 9) or radiographs ( n = 2). Conclusion This review identified limited randomized controlled trials, limited studies outside the knee, limited description of cell‐based treatments, and treatment protocols, along with inconsistent patient‐reported outcomes limited to 1 year in most studies. We propose establishing reporting guidelines in research on cell‐based therapies.

Abstract Background Isocitrate dehydrogenase (IDH)-mutant gliomas are infiltrative tumors with limited treatment options at recurrence. Early studies suggest IDH inhibition has therapeutic activity. We evaluated clinical outcomes associated with IDH-1 inhibitor ivosidenib in recurrent IDH-1-mutant gliomas and molecular predictors of response. Methods We retrospectively analyzed adults treated with ivosidenib from January 2021 to August 2025 with predominantly recurrent, pretreated CNS WHO grade 2-4 astrocytoma IDH-mutant or grade 2-3 oligodendroglioma, IDH-mutant and 1p/19q codeleted. Prior anticancer treatments, toxicities, enhancement status at ivosidenib initiation, and genomic alterations were collected. The primary endpoint was progression-free survival (PFS); secondary analyses evaluated associations between PFS, genomic complexity, and CDKN2A/B homozygous deletion. Results Ninety-two adults, median age 43, were included: 61 astrocytomas and 31 oligodendrogliomas, mostly grade 2. Before ivosidenib, 69 had surgery, 64 chemotherapy, and 64 radiation. Median time from diagnosis to ivosidenib was 4.8 years, with median of two prior progressions. Median treatment duration was 5.8 months. Median PFS was 15.2, 4.6, and 2.1 months for astrocytoma grades 2-4, 14.0 and 6.9 months for oligodendroglioma grades 2-3. Increasing genomic complexity was associated with shorter PFS in univariate analysis. Patients without CDKN2A/B homozygous deletion had median PFS of 19.2 months. Two patients with homozygous deletion had median PFS of 0.6 months. Conclusions In this retrospective analysis of predominantly recurrent IDH-1-mutant glioma, ivosidenib was associated with longer PFS in lower-grade, non-enhancing tumors that received little or no prior anticancer treatment and less complex genomic profiles. These findings support a context-dependent benefit of IDH inhibition in recurrent disease.

Abstract Purpose The purpose of this systematic review was to compare the inter‐ and intra‐observer reliability of Kellgren–Lawrence (KL) grading versus minimum joint space width (mJSW) measurement on knee radiographs, and to examine the influence of rater type, radiographic view and atlas‐based training. Methods A systematic search of PubMed/MEDLINE, Embase, Scopus and Web of Science was conducted through July 2025 following Preferred Reporting Items for Systematic reviews and Meta‐Analyses (PRISMA) guidelines. Studies reporting inter‐ and/or intra‐observer reliability for KL or mJSW on standard knee radiographs were included. Extracted data included kappa ( κ ) and intraclass correlation coefficient (ICC) values, rater expertise, radiographic view and atlas usage. Raters were categorized as expert, trainee/non‐expert or artificial intelligence (AI)‐assisted/automated. Results Twenty‐four studies (10,394 radiographs) met the inclusion criteria. Inter‐observer reliability was generally higher for mJSW (mean ICC = 0.82 ± 0.17) compared with KL (mean k = 0.72 ± 0.17), with similar trends observed for intra‐observer reliability. Experts and AI‐assisted systems outperformed trainees/non‐experts for both metrics. Reliability improved with semiflexed posteroanterior/Rosenberg views and atlas‐guided training. Conclusion This systematic review demonstrates that mJSW measurement generally shows higher reproducibility than KL grading. Reliability improved with expertise, standardized imaging and structured training, and AI‐assisted approaches performed comparably to expert raters. Quantitative mJSW should complement KL grading, particularly when combined with alignment measures, to enhance consistency and clinical relevance of radiographic assessment. Abstract INTRODUCTION Anti—amyloid‐β (Aβ) therapies are reshaping Alzheimer's disease (AD) management. Understanding changes in real‐world patterns of diagnostic testing and infusion chair usage is essential for optimizing access to care. METHODS Retrospective analysis of Mayo Clinic enterprise electronic health records (Jan 2019–Mar 2025) assessed trends in AD‐relevant brain imaging, fluid biomarkers, apolipoprotein E (APOE) testing, and lecanemab infusions. Rates of amyloid‐beta (Aβ) positivity by sex and age, APOE genotype frequencies, and lecanemab treatment initiation and discontinuation were evaluated. RESULTS Following national insurance coverage changes, lecanemab infusions grew by 110 infusions per quarter to 605 in Q1 2025. Aβ positron emission tomography scans increased (+22/quarter), cerebrospinal fluid biomarker orders declined (−25/quarter), and plasma p‐tau 217 orders rapidly increased (+238/quarter). Females were more likely to be Aβ positive ( p < 0.006). APOE ‐ε4 homozygotes were less likely to initiate lecanemab (HR = 0.11, p < 0.001). DISCUSSION The adoption of anti‐Aβ therapies coincided with a rapid shift in diagnostic workflows.

BACKGROUND In patients with drug-resistant epilepsy (DRE) who are not candidates for resective surgery, thalamic neuromodulation targeting structures such as the anterior nucleus of the thalamus (ANT), centromedian nucleus (CM), and pulvinar (PUL) has emerged as a promising therapy. The clinical investigation of these targets’ efficacy and side effect profiles is ongoing, complicating discussions between neurosurgeons, neurologists, and patients. The implementation of single-pulse and continuous stimulation protocols to conduct patient-specific side effect screens and generate prognostic biomarkers could offer guidance for target selection. OBSERVATIONS A 32-year-old male with extensive polymicrogyria and DRE underwent stereo-electroencephalography (SEEG) with multisite thalamic sampling to inform neuromodulation target selection. Single-pulse stimulation was conducted to screen for acute sensorimotor effects, followed by continuous (12- to 24-hour) stimulation using clinically relevant parameters. Intolerable paresthesia occurred during CM single-pulse stimulation, leading to its exclusion as a target. Single-pulse and continuous stimulation of the ANT and PUL were well tolerated. PUL stimulation offered superior suppression of epileptiform discharges during sleep. LESSONS These findings meaningfully informed the selection of the PUL as a neuromodulation target. This case illustrates the safety, feasibility, and utility of patient-specific single-pulse and continuous stimulation assessments during SEEG monitoring to evaluate efficacy and tolerability prior to chronic neuromodulation, particularly for novel thalamic targets. https://thejns.org/doi/10.3171/CASE25947

Abstract Non-small cell lung cancer (NSCLC) exhibits strikingly different responses to antiangiogenic therapies between its two major histologic subtypes, lung adenocarcinoma (LUAD) and squamous cell carcinoma (LUSC), suggesting an histotype-specific regulation of angiogenesis. Cancer-associated fibroblasts (CAFs) commonly exhibit an activated/myofibroblast-like phenotype in NSCLC, and are emerging as key modulators of tumor progression; however, their contribution to angiogenic control remains undefined. Here, we investigated angiogenesis and hypoxia signatures in NSCLC and integrated bulk RNA-seq, scRNA-seq, CAF secretome profiling, genetic perturbations, and functional in vitro and in vivo assays to dissect the histotype-dependent production of pro-angiogenic factors by CAFs. We observed greater angiogenesis and reduced necrosis/hypoxia in LUAD compared to LUSC across multiple patient cohorts. The LUAD-CAF secretome was primed for angiogenesis through SMAD3-dependent overproduction of key regulators, most notably TIMP-1 and VEGF-A. We also uncovered a previously unrecognized role for TIMP-1 in promoting endothelial hyper-branching. In contrast, LUSC-CAFs displayed attenuated angiogenic activity despite robust HIF-1α upregulation and an hypoxia-associated transcriptional program, due to their epigenetic repression of SMAD3 and compensatory increase in SMAD2. Collectively, these results reveal that CAFs critically shape the distinct angiogenic landscapes of LUAD and LUSC through opposing SMAD2/3 regulation of TIMP-1, VEGF-A, and hypoxia signaling. These results further highlight the therapeutic potential of targeting stromal SMAD3/TIMP-1 in LUAD or microenvironmental stressors such as hypoxia and acidosis in LUSC. In addition, these findings provide a biological framework for understanding histotype-specific patterns of dissemination, immune evasion, and response to antiangiogenic therapies in NSCLC.
