Abstract We present a comprehensive global analysis of genetic variants associated with autosomal-dominant Alzheimer’s disease (ADAD). A total of 550 variants in the APP, PSEN1 and PSEN2 genes were identified, of which 279 were classified as pathogenic or likely pathogenic based on American College of Medical Genetics and Genomics and the Association for Molecular Pathology criteria, utilizing data from the Dominantly Inherited Alzheimer Network (DIAN), literature and public databases. Symptomatic age at onset (AAO) data were estimated for 227 of these variants, allowing detailed characterization of their frequency, pathogenicity and AAO. Importantly, 226 variants met eligibility criteria for inclusion in disease-modifying clinical trials. Furthermore, we demonstrated the predictive value of mean variant AAO and parental AAO in predicting symptomatic AAO, validated against converters who became symptomatic during follow-up in the DIAN Observational Study. This dataset provides critical insights into the global landscape of ADAD and reveals the genetic and AAO heterogeneity of ADAD variants while refining variant trial eligibility criteria.
Delphi-Based Global Consensus on Adopting Endothelial Keratoplasty: An Endothelial Keratoplasty Learners Group Initiative
Purpose: To identify areas of consensus among experts on the performance of endothelial keratoplasty by using a modified Delphi approach, to help create a framework for novice surgeons to adopt these procedures. Methods: Thirty-one international experts in endothelial keratoplasty participated. Two rounds of electronic survey were followed by a hybrid, virtual meeting. Consensus was set at 75%, and results with agreement between 70% and 75% were deemed as achieving near consensus. Results: A consensus was reached for 98 statements covering the preoperative, intraoperative, and postoperative aspects of Descemet membrane endothelial keratoplasty (DMEK) and Descemet stripping endothelial keratoplasty/Descemet stripping automated endothelial keratoplasty. Four statements achieved near consensus, and consensus could not be achieved for 11 statements. For DMEK, the panel supported a peel technique to prepare tissue for endothelium out DMEK, implanted via an injector and supported by a near full air/gas fill as a baseline procedure onto which more advanced techniques can be built. DMEK tissue should be marked to ensure correct orientation. An inferior peripheral iridotomy should be used to prevent pupil block when a near full air/gas fill in used in endothelial keratoplasty (EK). Descemet stripping automated endothelial keratoplasty was considered preferable to Descemet stripping endothelial keratoplasty where access to microkeratome preparation was available. Conclusions: The Delphi process allowed areas of consensus on the performance of EK to be established by a group of international experts. The statements generated are a helpful framework for novice surgeons learning EK. Further research is needed to help determine what specific tomographic features indicate EK, when guttae are considered visually significant and how to approach combined aphakia and endothelial dysfunction.
Largest Real-World Evaluation of Treatment Regimens and Clinical Outcomes Among Patients with Acute Promyelocytic Leukemia in the US: Data from the Command Consortium
Background: Acute promyelocytic leukemia (APL) is one of the most treatable leukemias in the current era, with cure rates upwards of 90%. Early mortality due to coagulopathies often seen at diagnosis is a major driver of adverse outcomes. Prompt initiation of all-trans retinoic acid (ATRA) is crucial for suspected APL, as it is known to reverse these coagulopathies. Chemotherapy-free protocols consisting of arsenic trioxide (ATO) and ATRA have become the standard of care for low-risk APL with high cure rates and reduced hematologic toxicities. For high-risk disease, there are multiple treatment options that have not been compared. Since APL is rare, there is a lack of real-world data for this disease. This retrospective review aimed to assess the outcomes of APL patients (pts) across multiple institutions and to examine the most common induction regimens, characteristics linked to early (30-day) mortality, and supportive care strategies. Methods: This retrospective study involved 9 institutions in the US and was conducted through the Consortium on Myeloid Malignancies and Neoplastic Diseases (COMMAND). Data included pts admitted from 8/5/2006 through 2/3/2024 with a confirmed diagnosis of APL. Each institution received a data-collection form and contributed data on APL pts from presentation to last follow-up. Key data categories included patient demographics, presenting laboratory and molecular data, treatment regimen, supportive care, and patient outcomes. Results: Data for 412 pts were identified with a median age of 51 (range 17-90) and 49% were female. Cardiovascular comorbidities were present in 44% of pts and 16% had a known pulmonary comorbidity. At presentation 109 (27%) had high risk-disease, defined as a white blood cell count (WBC) greater than 10 x 109/L. A FLT3 mutation was seen in 50% of tested pts, WT1 (22%), KRAS (3.6%), and NRAS (6.5%). The median WBC at presentation was 2 x 109/L (range: 0-166 x 109/L) and the median platelet count was 27 x 109/L (range: 2.0-315 109/L). Twenty-six percent presented with grade 2+ thrombosis, bleeding, or both. A higher percentage of pts with high-risk vs. low-risk disease presented with grade 2+ thrombosis, bleeding, or both (43% vs. 20%), DIC (60% vs. 36%), and mutated FLT3 (81% vs. 37%). The majority of pts received a chemotherapy-free induction regimen consisting of ATRA plus ATO. It was determined that 36 (8.7%) pts received induction with intermittent ATO plus ATRA (Burnett AK, Lancet Oncol 2015), 276 (67%) received daily ATO plus ATRA (Ravandi F, J Clin Oncol 2009 and Lo Coco, NEJM 2013), and 70 (17%) received a regimen inclusive of an anthracycline plus ATRA. Eleven pts did not start an induction regimen other than single-agent ATRA due to early death, while 19 had their regimen classified as “other” because they did not follow a conventional induction path. In total, 295/358 (82%) received ATRA within 24 hours of admission. At least 1 dose of gemtuzumab ozogamicin was given to 58 (14%) pts, of which 35 (60%) were considered high risk. The incidence of differentiation syndrome (DS) was 37% (n=154). Among the 243 pts who received steroid prophylaxis, 30% developed DS, compared to 52% of those who did not receive prophylaxis. Among the 360 pts with at least 1 available PCR result after starting induction, 94% achieved PCR-negativity. The 30-day mortality rate was 6.6%. On univariate analysis, presenting variables associated with a higher 30-day mortality include older age, diabetes, cardiovascular comorbidities, thrombosis or bleeding, DIC, eGFR < 60, elevated total bilirubin and D-dimer levels, and high-risk disease. The median overall survival (OS) of the cohort was not reached, and the 5- and 10-year OS was 88% (95% CI: 84-91%) and 79% (95% CI: 72-87%) respectively. The cumulative incidence of relapse at 3 years was 2.7% (95% CI: 1.4 - 5.1%), with no additional relapses observed. The incidence of CNS relapse at 3 years was 0.9% (95% CI: 0.3% - 2.8%), with 2 of the 4 pts classified as high-risk. Conclusion: This is the largest real-world evidence data collection in the US evaluating the outcome of pts with APL in the current era, inclusive of ATO as front-line induction. Outcomes for this disease remain extremely positive, with low 30-day mortality rates noted in this study along with durable OS rates and low incidence of relapse. Additional information on outcomes, safety, and supportive care will be presented at the meeting.
Targeting lysosomal quality control as a therapeutic strategy against aging and diseases
AbstractPreviously, lysosomes were primarily referred to as the digestive organelles and recycling centers within cells. Recent discoveries have expanded the lysosomal functional scope and revealed their critical roles in nutrient sensing, epigenetic regulation, plasma membrane repair, lipid transport, ion homeostasis, and cellular stress response. Lysosomal dysfunction is also found to be associated with aging and several diseases. Therefore, function of macroautophagy, a lysosome‐dependent intracellular degradation system, has been identified as one of the updated twelve hallmarks of aging. In this review, we begin by introducing the concept of lysosomal quality control (LQC), which is a cellular machinery that maintains the number, morphology, and function of lysosomes through different processes such as lysosomal biogenesis, reformation, fission, fusion, turnover, lysophagy, exocytosis, and membrane permeabilization and repair. Next, we summarize the results from studies reporting the association between LQC dysregulation and aging/various disorders. Subsequently, we explore the emerging therapeutic strategies that target distinct aspects of LQC for treating diseases and combatting aging. Lastly, we underscore the existing knowledge gap and propose potential avenues for future research.
Weight gain in Chinese youth during a 4-month COVID-19 lockdown: a retrospective observational study
Objectives To observe the weight change in Chinese youth during a 4-month COVID-19 lockdown, and the association between weight change and mental health, physical activity and sedentary time changes, and dietary habits. Design A retrospective observational study. Settings Two universities located in Zhejiang and Hunan provinces, China. Participants This study enrolled 12 889 college students whose body weight was measured before the lockdown (1 December 2019–20 January 2020) at the two universities, and reported their weight measured at home or community after the end of the lockdown (1–23 May 2020) via an online follow-up questionnaire. Primary and secondary outcome measures The primary outcome was the weight change in Chinese youth during a 4-month lockdown resulting from the COVID-19 pandemic. The secondary outcomes were the relationships of weight change to COVID-19-related stress, depression, anxiety, physical activity and sedentary time changes, and dietary habits. Results Participants’ ages ranged from 17 to 27 years ( M =19, SD=1) with 80.2% identified as female. The average absolute and relative changes in body weight were 2.6 (95% CI 2.0 to 3.2)) kg and 4.2% (95% CI 4.0% to 4.3%) for men, and 2.1 (1.9 to 2.4) kg and 4.2% (95% CI 3.9% to 4.4%) for women. An increase in overweight and obese individuals according to Asian cut-off points as a demographic percentage by 4.5% and 2.7% and 4.8% and 3.4% in men and women, respectively ( P <0.001), was observed. Weight gain was significantly associated with increased sedentary time and an increase in COVID-19-related stress and depression score. Conclusion The present study’s results suggest that the risk of weight gain in Chinese youth during the lockdown increased and that strategies to decrease sedentary time and improve mental health may be warranted to mitigate weight gain during and after the COVID-19 pandemic.

Background The COVID-19 pandemic has led to a spike in deleterious mental health. This dual-center retrospective cross-sectional study assessed the prevalence of depression in young adults during this pandemic and explored its association with various physical fitness measures. Methods This study enrolled 12,889 (80% female) young adults (mean age 20 ± 1) who performed a National Student Physical Fitness battery from December 1st, 2019, to January 20th, 2020, and completed a questionnaire including Beck’s Depression Inventory in May 2020. Independent associations between prior physical fitness and depression during the pandemic were assessed using multivariable linear and binary logistic regressions accordingly, covariates including age, dwelling location, economic level, smoking, alcohol, living status, weight change, and exercise volume during the pandemic. Sex- and baseline stress-stratified analyses were performed. Results Of the study population 13.9% of men and 15.0% of women sampled qualified for a diagnosis of depression. After multivariable adjustment, anaerobic (mean change 95% CI −3.3 [−4.8 to 1.8]) aerobic (−1.5 [−2.64 to −0.5]), explosive (−1.64 [−2.7 to −0.6]) and muscular (−1.7 [−3.0 to −0.5]) fitness were independently and inversely associated with depression for the overall population. These remained consistent after sex- and baseline stress-stratification. In binary logistic regression, the combined participants with moderate, high or excellent fitness also showed a much lower risk compared to those least fit in anaerobic (odd ratio (OR) 95% CI 0.68 [0.55–0.82]), aerobic (0.80 [0.68–0.91]), explosive (0.72 [0.61–0.82]), and muscular (0.66 [0.57–0.75]) fitness. Conclusions These findings suggest that prior physical fitness may be inversely associated with depression in young adults during a pandemic.

An increase in overweight and obese individuals according to Asian cut-off points as a demographic percentage by 4.5% and 2.7% and 4.8% and 3.4% in males and females respectively (P<0.001). Weight change was associated with sedentary time, COVID-19 induced stress, and depression score. Conclusion: These findings suggest that limiting sedentary time during lockdowns and strategies aimed t improve lifestyle habits and psychological health may be warranted to mitigate weight-gain during the pandemic.Registration: Registered with ChiCTR, registration no.: ChiCTR2000033514
The mutational landscape of spinal chordomas and their sensitive detection using circulating tumor DNA
Abstract Background Chordomas are the most common primary spinal column malignancy in the United States. The aim of this study was to determine whether chordomas may be detected by evaluating mutations in circulating tumor DNA (ctDNA). Methods Thirty-two patients with a biopsy-confirmed diagnosis of chordoma had blood drawn pre-operatively and/or at follow-up appointments. Mutations in the primary tumor were identified by whole exome sequencing and liquid biopsy by ddPCR and/or RACE-Seq was used to detect one or more of these mutations in plasma ctDNA at concurrent or later time points. Results At the time of initial blood draw, 87.1% of patients were ctDNA positive (P <.001). Follow-up blood draws in twenty of the patients suggest that ctDNA levels may reflect the clinical status of the disease. Patients with positive ctDNA levels were more likely to have greater mutant allele frequencies in their primary tumors (P = .004) and undergo radiotherapy (P = .02), and the presence of ctDNA may correlate with response to systemic chemotherapy and/or disease recurrence. Conclusions Detection of ctDNA mutations may allow for the detection and monitoring of disease progression for chordomas.
COVID-19 Induced Stress, Lifestyle Changes and Weight-gain in Youth during a 4-Month Lockdown: A Prospective Cohort Study
Abstract The COVID-19 (coronavirus disease-19) has resulted in nationwide lockdowns, cessation of school and work, and social distancing. Reducing the negative impacts, such as weight-gain, has garnered global attention. In this dual-center prospective cohort study of 12,889 students, we assessed weight-gain in a youth during the 4-month COVID-19 lockdown, and examined the associations between weight-gain and COVID-19 induced stress, depression and self-reported physical activity, dietary habits and sedentary time. Participants gained an average of 2.6 kg (95% CI: 2.0 to 3.2 kg) for males and 2.1 kg (1.9 to 2.4 kg) for females. An increase in overweight and obese individuals as a demographic percentage by 4.5% and 2.7% and 4.8% and 3.4% in males and females respectively (P<0.001). Weight change was independently associated with severe COVID-19 induced stress, sedentary time, and depression score. Techniques to relieve COVID-19 induced stress, reduce sedentary time through increased physical activity, and improve dietary habits in youth may be warranted to help prevent and/or reverse weight-gain associated with the lockdown period of COVID-19.Registration: Registered with ChiCTR, registration no.: ChiCTR2000033514
