Genome-wide association studies have identified >50 common variants associated with kidney function, but these variants do not fully explain the variation in eGFR. We performed a two-stage meta-analysis of associations between genotypes from the Illumina exome array and eGFR on the basis of serum creatinine (eGFRcrea) among participants of European ancestry from the CKDGen Consortium ( n Stage1 : 111,666; n Stage2 : 48,343). In single-variant analyses, we identified single nucleotide polymorphisms at seven new loci associated with eGFRcrea ( PPM1J , EDEM3, ACP1, SPEG, EYA4, CYP1A1 , and ATXN2L ; P Stage1 <3.7×10 −7 ), of which most were common and annotated as nonsynonymous variants. Gene-based analysis identified associations of functional rare variants in three genes with eGFRcrea, including a novel association with the SOS Ras/Rho guanine nucleotide exchange factor 2 gene, SOS2 ( P =5.4×10 −8 by sequence kernel association test). Experimental follow-up in zebrafish embryos revealed changes in glomerular gene expression and renal tubule morphology in the embryonic kidney of acp1- and sos2 -knockdowns. These developmental abnormalities associated with altered blood clearance rate and heightened prevalence of edema. This study expands the number of loci associated with kidney function and identifies novel genes with potential roles in kidney formation.

Abstract B41: Cell-contact dependent epithelial-stromal cross talk, including a modulation of stromal PDGFR expression, drives the progression of early-stage breast cancer lesions
Abstract We have investigated the distribution of PDGFR-a and -b in the stroma of normal breast tissue, ductal carcinoma in situ (DCIS) and invasive ductal carcinoma (IDC) by immunohistochemistry. Both receptors were expressed by fibroblasts of normal breast tissue. While the PDGFRb remained high during cancer progression, the PDGFRa was lost in most of the DCIS samples and not detectable in IDC. The patient group with PDGFRa(low)/PDGFRb(high) fibroblasts had the highest risk for local recurrences or metastasis. The characteristic loss of stromal PDGFRa expression was also observed in the PyMT mouse breast cancer model. In vitro co-culture experiments confirmed a tumor cell-induced differentiation of fibroblasts, resulting in a fibroblast population marked by PDGFRa(low)/PDGFRb(high) expression. We could identify the interaction of Jagged-1 expressed by the tumor cells with Notch-2 expressed on the fibroblasts, as an essential activator of TGFb signaling in fibroblasts. Gene expression analysis and reporter-gene assays of sorted cells from co-cultures revealed, that fibroblasts switch from a more anti-tumorigenic signaling (expression of BMPs and Wnt antagonists) towards a pro-tumorigenic signaling (expression of BMP inhibitors, TGFb and Wnt ligands). Co-cultured tumor cells show an increased migratory activity and the induction of a mesenchymal phenotype. Inhibition of Notch or TGF signaling in fibroblasts could block these phenotypes. To investigate the impact of fibroblasts on the progression of DCIS to IDC, a DCIS xenograft model was used. Co-injection of MCF10.DCIS cells with human breast fibroblasts, expressing both PDGFRs, into the mouse mammary fat pad resulted in an earlier tumor take with faster tumor growth. The corresponding co-injection experiments with fibroblast and tumor cells, that were depleted of Notch2 respectively Jagged-1 expression by CRISPR/CAS technology is currently ongoing. Collectively the study identifies a potentially druggable paracrine cross-talk between epithelial and stromal cells, which promotes DCIS progression and involves a Notch mediated up-regulation of TGFb signaling. The study also suggests that loss of PDGFRa marks a subset of fibroblasts with specific tumor-promoting activities. New Potential Biomarker Proteins for Alcoholic Liver Disease Identified by a Comparative Proteomics Approach
Background The study compared downstream coronary and conduit disease progression in the left anterior descending coronary artery treated with coronary artery bypass grafting using the left internal mammary artery ( LIMA ) versus percutaneous coronary intervention with bare metal stent ( BMS ) or drug eluting stent ( DES ). Methods and Results A total of 12 301 consecutive patients underwent isolated primary coronary revascularization, of which 2386 met our inclusion criteria (Percutaneous coronary intervention, n=1450; coronary artery bypass grafting, n=936). Propensity score analysis matched 628 patients, of which 468 were treated to the left anterior descending with coronary artery bypass grafting with LIMA (n=314), percutaneous coronary intervention with BMS (n=94), and DES (n=60). Coronary angiograms were analyzed by quantitative coronary angiography ( QCA ; n=433). Cumulative downstream coronary and conduit disease progression were estimated by Kaplan–Meier method and effect of treatment type by Cox proportional hazard models. Patients treated with LIMA had significantly lower risk of downstream coronary disease progression at follow‐up angiogram compared with BMS and DES (hazard ratio [ HR ] [95% CI ], 0.34; [0.20–0.59]; P =0.0002; and HR [95% CI ], 0.39; [0.20–0.79]; P =0.01, respectively). LIMA was associated with a lower risk of conduit disease progression compared to BMS and DES ( HR [95% CI ], 0.18; [0.12–0.28]; P <0.001; and HR [95% CI ], 0.27; [0.16–0.46]; P <0.001, respectively). BMS was associated with higher HR for downstream coronary and conduit disease progression compared with DES , but the difference did not reach statistical significance ( HR [95% CI], 1.13; [0.57–2.36]; P =0.73; and HR [95% CI ], 1.46; [0.88–2.50]; P =0.14, respectively). Conclusions LIMA grafting to left anterior descending is associated with significantly lower risk of downstream coronary and conduit disease progression compared to percutaneous coronary intervention with BMS and DES .
A Pharmacological Chaperone Molecule Induces Cancer Cell Death by Restoring Tertiary DNA Structures in Mutant hTERT Promoters
Although the majority of giant cell tumors (GCTs) of the bone occur in adult patients, occasionally they arise in the pediatric population. In this setting they may be mistaken for tumors more commonly seen in this age group, including osteosarcoma, aneurysmal bone cyst, and chondroblastoma. All cases of primary GCT of the bone arising in patients 18 years and below were retrieved from our institutional archives and examined with emphasis on the evaluation of various morphologic patterns. Clinical/radiologic records were reviewed when available. Analysis for H3F3A/H3F3B mutations was performed in a subset of cases. Sixty-three (of 710) patients treated at our institution for GCT were 18 years of age and below. The following morphologic patterns were identified: fibrosis (31 cases, 49%), reactive-appearing bone (26, 41%), cystic change (7, 11%), foamy histiocytes (6, 10%), secondary aneurysmal bone cyst (3, 5%), and cartilage (2, 3%). Infarct-like necrosis was present in 17 tumors (27%), and the mitotic rate ranged from 0 to 35 mitoses/10 high-power fields (median 5 mitoses/10 high-power field). Follow-up information (n=55; 6 mo to 69.6 y; median, 11.6 y) showed 21 patients with local recurrence (38%) and 2 patients with lung metastasis (4%). Polymerase chain reaction with sequencing showed that 5 of 5 tested cases harbored H3F3A mutations. In summary, GCT arising in the pediatric population is rare, representing 9% of GCTs seen at our institution. The morphologic spectrum of these tumors is broad and similar to that seen in patients above 18 years of age. It is important to recognize that matrix formation may be observed in GCT, including reactive-appearing bone and cartilage, as well as areas of fibrosis mimicking osteoid production, to avoid misclassification as osteosarcoma or other giant cell–rich lesions common in children.
