Recent reports have suggested a high prevalence of olfactory bulb synucleinopathy in Lewy body disorders but there have been no estimates of the sensitivity or specificity of this as a diagnostic test. We performed α‐synuclein immunostaining on paraffin‐embedded sections of olfactory bulb from a series of neuropathologically‐diagnosed subjects from the Sun Health Research Institute Brain Donation Program/Arizona Parkinson's Disease Consortium. Included in the series were 76 normal elderly individuals, 8 subjects with incidental Lewy body disease (ILBD), 63 with Parkinson's disease (PD) and 79 with dementia with Lewy bodies (DLB). Sections were stained with a sensitive method employing pretreatment with proteinase K. The ratios of positive to negative olfactory bulb staining for each diagnostic category were: normal 5/71; ILBD 8/0; PD 57/6; DLB 74/5. An α‐synuclein‐positive olfactory bulb has 92% sensitivity and 93% specificity for the diagnosis of PD versus normal while for DLB it is 94% and 93%, respectively. As all 8 cases of ILBD had a positive olfactory bulb it appears that olfactory bulb synucleinopathy may also be a reliable marker of presymptomatic Lewy body disorders. Twelve cases (6 M, 6F; mean age 87.9) with incidental Lewy bodies (no clinical diagnosis of parkinsonism or dementia) had medical histories of supraventricular arrhythmia (6/12), dysphagia (5/12), depression (5/12) and falls (4/12). Standardized movement disorder assessments were performed antemortem on 6 cases. The mean Part III UPDRS score was 7 (range 2–14). Motor findings included kinetic tremor (5/6), rest tremor (3/6), postural instability (3/6), postural tremor (2/6), and voice tremor (2/6). Of these 6 cases one was considered to have a normal movement exam, 2 were diagnosed as essential tremor, and 3 as nonspecific tremor disorder. Mean MMSE score in 7 cases was 26.6 (range 23–30). Medical records review revealed 4/12 cases had cognitive impairment not meeting criteria for dementia. Of these 12 cases, 5 had received standardized cognitive assessments: 3 were categorized as cognitively normal and 2 as mild cognitive impairment. Our series of incidental Lewy body disease cases reveal a range of neurologic findings including tremor and cognitive impairment. Antemortem standardized evaluations suggest there may be early clinical markers for Lewy body disorders. BackgroundMultiple lines of evidence suggest that cardiovascular co‐morbidities hasten the onset of Alzheimer's disease (AD) or accelerate its course.MethodsTo evaluate the utility of cerebral vascular physical function and/or condition parameters as potential systemic indicators of AD, transcranial Doppler (TCD) ultrasound was used to assess cerebral blood flow and vascular resistance of the 16 arterial segments comprising the circle of Willis and its major tributaries.ResultsOur study showed that decreased arterial mean flow velocity and increased pulsatility index are associated with a clinical diagnosis of presumptive AD. Cerebral blood flow impairment shown by these parameters reflects the global hemodynamic and structural consequences of a multifaceted disease process yielding diffuse congestive microvascular pathology, increased arterial rigidity, and decreased arterial compliance, combined with putative age‐associated cardiovascular output declines.ConclusionsTCD evaluation offers direct physical confirmation of brain perfusion impairment and might ultimately provide a convenient and a noninvasive means to assess the efficacy of medical interventions on cerebral blood flow or reveal incipient AD. In the near term, TCD‐based direct assessments of brain perfusion might offer the prospect of preventing or mitigating AD simply by revealing patients who would benefit from interventions to improve circulatory system function.

A recently identified variant within the fat mass and obesity-associated ( FTO ) gene is carried by 46% of Western Europeans and is associated with an ~1.2 kg higher weight, on average, in adults and an ~1 cm greater waist circumference. With >1 billion overweight and 300 million obese persons worldwide, it is crucial to understand the implications of carrying this very common allele for the health of our aging population. FTO is highly expressed in the brain and elevated body mass index (BMI) is associated with brain atrophy, but it is unknown how the obesity-associated risk allele affects human brain structure. We therefore generated 3D maps of regional brain volume differences in 206 healthy elderly subjects scanned with MRI and genotyped as part of the Alzheimer's Disease Neuroimaging Initiative. We found a pattern of systematic brain volume deficits in carriers of the obesity-associated risk allele versus noncarriers. Relative to structure volumes in the mean template, FTO risk allele carriers versus noncarriers had an average brain volume difference of ~8% in the frontal lobes and 12% in the occipital lobes—these regions also showed significant volume deficits in subjects with higher BMI. These brain differences were not attributable to differences in cholesterol levels, hypertension, or the volume of white matter hyperintensities; which were not detectably higher in FTO risk allele carriers versus noncarriers. These brain maps reveal that a commonly carried susceptibility allele for obesity is associated with structural brain atrophy, with implications for the health of the elderly.

BackgroundWe evaluated the amounts of amyloid beta (Aβ)) peptides in the central nervous system (CNS) and in reservoirs outside the CNS and their potential impact on Aβ plasma levels and Alzheimer's disease (AD) pathology.MethodsAmyloid β levels were measured in (1) the plasma of AD and nondemented (ND) controls in a longitudinal study, (2) the plasma of a cohort of AD patients receiving a cholinesterase inhibitor, and (3) the skeletal muscle, liver, aorta, platelets, leptomeningeal arteries, and in gray and white matter of AD and ND control subjects.ResultsPlasma Aβ levels fluctuated over time and among individuals, suggesting continuous contributions from brain and peripheral tissues and associations with reactive circulating proteins. Arteries with atherosclerosis had larger amounts of Aβ40 than disease‐free vessels. Inactivated platelets contained more Aβ peptides than activated ones. Substantially more Aβ was present in liver samples from ND patients. Overall, AD brain and skeletal muscle contained increased levels of Aβ.ConclusionsEfforts to use plasma levels of Aβ peptides as AD biomarkers or disease‐staging scales have failed. Peripheral tissues might contribute to both the circulating amyloid pool and AD pathology within the brain and its vasculature. The wide spread of plasma Aβ values is also due in part to the ability of Aβ to bind to a variety of plasma and membrane proteins. Sources outside the CNS must be accounted for because pharmacologic interventions to reduce cerebral amyloid are assessed by monitoring Aβ plasma levels. Furthermore, the long‐range impact of Aβ immunotherapy on peripheral Aβ sources should also be considered.
