Background/Objectives: In the United States, HF prevalence is projected to progressively rise by 2030. Prior research suggests a strong association between reduced sleep duration and increased cardiovascular disease and HF risk. This study introduces an alternative parameter, the weekend sleep recovery (WSR), measured by the weekend-to-weekday sleep duration ratio (SDR), to evaluate its association with HF risk. Methods: We conducted a cross-sectional analysis of NHANES (National Health and Nutrition Examination Survey) 2017–2023 to examine self-reported sleep patterns. Participants were classified as WSR (SDR > 1) or non-WSR (SDR ≤ 1). Multivariate logistic regression assessed the association between WSR and HF, adjusting for demographics and comorbidities. Results: Among 8320 participants included in the fully adjusted analysis, WSR was associated with lower odds of HF compared with non-WSR (adjusted OR 0.73, 95% CI 0.55–0.96; p = 0.026). A significant interaction was observed between WSR and weekday sleep duration (P for interaction = 0.003), whereas no interaction was found with weekend sleep duration. In exploratory subgroup analyses, nominally significant associations were observed in several clinical subgroups; however, after correction for multiple comparisons, only participants without dyslipidemia retained statistical significance. No significant effect modification by race/ethnicity was observed (P for interaction = 0.436). Conclusions: Weekend sleep recovery was associated with lower odds of HF in this cross-sectional study. The association varied according to weekday sleep duration but was generally consistent across racial/ethnic groups. Given the observational design, these findings should be interpreted as associative rather than causal and warrant confirmation in prospective studies.

Background/Objective: Clostridioides difficile infection (CDI) is a common healthcare-associated ailment, presenting major health and economic challenges, especially for the elderly. Despite its prevalence, comprehensive data about CDI’s impact on the elderly are limited. Methods: This study used the Global Burden of Disease Study 2019 data to analyze CDI trends from 2000 to 2019, considering factors like sex, region, and sociodemographic index (SDI). Results: This study revealed that CDI caused approximately 18,181 deaths and 252,709 disability-adjusted life years (DALYs) among the elderly worldwide. The Americas showed the highest CDI burden, while the Eastern Mediterranean saw the steepest rate increase from 2000 to 2019. Regions with a high SDI also displayed substantial CDI impact. Conclusions: The escalating burden of CDI in the elderly, especially in high-SDI areas and the Americas, emphasizes an urgent need for targeted public health strategies.

Abstract INTRODUCTION Esophagogastroduodenoscopy is safe and beneficial in elderly patients. However, the association of dementia, which is a growing cause of mortality in the elderly, remains unexplored. We aim to understand the impact of dementia when EGD is utilized, in the elderly population group. METHODS We utilized the 2020 National Inpatient Sample to investigate records admitted with EGD utilization identified through ICD-10 CM codes. We divided records by the presence of dementia. Adjusted odds ratios (aORs) for predefined outcomes were determined using multivariable logistic and linear regression models, adjusting for comorbidities. The primary outcome assessed was inpatient mortality, while secondary outcomes included cost and length of stay, as well as other inpatient complications. RESULTS We identified 507,829 hospitalizations with ages more than 60. The mean age was 74.5 and males constituted 49.9%. In a survey multivariable logistic and linear regression model adjusting for patient and hospital factors, the presence of dementia is associated with lower inpatient mortality (aOR 0.79, 95%CI (0.68, 0.91), p = 0.002), lower hospitalization costs (beta coefficient -1,958, 95%CI (-2,653, -1,353), p <0.001), lower mechanical ventilation usage (aOR 0.87, 95%CI(0.78, 0.97), p < 0.001) but higher odds of mental status change (aOR 2.24, 95%CI (2.09, 2.40), p < 0.001) and longer length of stay (beta coefficient 0.52, 95%CI (0.30, 0.74), p <0.001),. Post-propensity matching revealed similar patterns. CONCLUSION The presence of dementia is associated with a lower risk of inpatient mortality, and other clinical outcomes, in hospitalization with EGD utilizations. Etiologies behind this relationship should be explored to understand this inverse relationship.
