Abstract Psychiatry has a long and controversial history, and its legacy is often questioned in the arts. But how is psychiatry used by filmmakers, what do they think of mental illness, and how do they use it? What is their frame of mind? Psychoanalytically? Biologically? Sociologically or anthropologically? Cinema has not only produced a full spectrum of elements of mental illness but also showed its treatment (or lack thereof). Frames of Minds traces the historical display of interpreted neuropsychiatry and explains why it is a story about medicine and why it should interest us. Patterns and paradigm shifts in neuropsychiatry over the last century have emerged over the decades, and mental disorders on screen became more specific. The influence of the anti-psychiatry movement in documentary filmmaking was evident. The takeaway is that filmmakers like to break the rules and take chances with the awkwardness of mental illness and psychotic breakdown.

Exchanges in medical practice are necessary for training. The use of movies for promoting medical practice was introduced in the late 19th century. The authors analyzed an unidentified movie titled Trepanation for Rolandic Zone Tumor (Trépanation pour tumeur de la zone rolandique) stored at the Établissement de Communication et de Production Audiovisuelle de la Défense of the French Ministry of Armed Forces. This silent black-and-white movie, lasting 15 minutes and depicting the removal of a meningioma, did not contain information or a legend. The surgical tools used were those developed by Thierry de Martel. The furniture and interior design shown in the film corresponded with the Vercingétorix Clinic that this surgeon had acquired in Paris. A publication from 1922 contained 14 pictures taken from the movie presented in this paper and referred to a movie directed in 1911 by Thierry de Martel. This is strong circumstantial evidence that the film was directed and the surgery was performed by Thierry de Martel at the Vercingétorix Clinic in Paris, France, in 1911 while using the technology of the Gaumont company. This is a contemporary testimony to what surgical practice was over a century ago, and it illustrates how movies were, and remain, a unique way to learn and teach medicine.

AbstractWhen progressive and severe, myasthenia gravis and Guillain–Barré syndrome may have the potential for fatal and unfavorable clinical outcomes. Regardless of important differences in their clinical course, the development of weakness of oropharyngeal muscles and respiratory failure with requirement of mechanical ventilation is the main driver of poor prognosis in both conditions. The need for prolonged mechanical ventilation is particularly relevant because it immobilizes the patient and care becomes extraordinarily complex due to daily risks of systemic complications. Additionally, patients with myasthenia gravis often require long-term immunosuppressive treatments with associated toxicity and infectious risks. Unlike myasthenia gravis, the recovery period is prolonged in Guillain–Barré syndrome, but often favorable, even in the more severely affected patients. Outcome, for a large part, is determined by expert neurocritical care.

Delirium is common in hospitalised patients, and there is currently no specific treatment. Identifying and treating underlying somatic causes of delirium is the first priority once delirium is diagnosed. Several international guidelines provide clinicians with an evidence-based approach to screening, diagnosis and symptomatic treatment. However, current guidelines do not offer a structured approach to identification of underlying causes. A panel of 37 internationally recognised delirium experts from diverse medical backgrounds worked together in a modified Delphi approach via an online platform. Consensus was reached after five voting rounds. The final product of this project is a set of three delirium management algorithms (the Delirium Delphi Algorithms), one for ward patients, one for patients after cardiac surgery and one for patients in the intensive care unit.
