ABSTRACT Large symptomatic splenic pseudocysts, though rare, should be considered in patients with persistent left upper abdominal pain as we report in our case. Imaging enables diagnosis, and laparoscopic spleen‐preserving cyst decompression and deroofing provide a safe, effective treatment, relieving symptoms while maintaining splenic function and avoiding the morbidity associated with splenectomy and its long‐term complications.
