Vitamin D supplementation of breastfed infants is recommended because breast milk levels are low. We sought to determine if maternal supplementation would increase breast milk levels to achieve sufficiency in their infants. Forty mothers with exclusively breastfed infants were randomized to receive oral cholecalciferol (vitamin D3) 5000 IU/d for 28 days or 150,000 IU once. Maternal serum, breast milk and urine were collected on days 0, 1, 3, 7, 14 and 28 and infant serum on days 0 and 28. In mothers receiving daily vitamin D, serum and breast milk cholecalciferol attained steady levels of 18 and 8 ng/ml, respectively, from day 3 through 28. In mothers given the single dose, serum and breast milk cholecalciferol peaked at 160 and 40 ng/ml, respectively at day 1, before rapidly declining. Breast milk and maternal serum cholecalciferol were related (r=0.87). Infant serum 25OHD (mean±SD) increased from 17±13 to 39±6 ng/ml in the single dose group and from 16±12 to 39±12 ng/ml in the daily dose group (P= 0.88). All infants achieved sufficient 25OHD serum levels (≥20ng/mL). This study suggests that maternal supplementation can achieve sufficient infant serum levels of 25OHD via breastfeeding. A large single dose of vitamin D was equivalent to daily dosing for optimizing infant vitamin D status. This research was funded by the National Center for Research Resources and Mayo Clinic. Background: Decreased testosterone (TTST) and sex hormone binding globin (SHBG) contribute to development of cardiometabolic disorders in obese men. We aimed to determine the effect of weight gain and weight loss on TTST and SHBG in healthy men and to examine their relationship with endothelial function. Method: 25 normal weight healthy men underwent an overfeeding protocol to gain 5% body weight over 8 weeks. Following weight gain, they underwent 8 weeks of weight loss. TTST, SHBG and endothelial function (brachial artery flow‐mediated dilation, FMD) were measured at baseline (T1), after weight gain (T2) and after weight loss (T3). Results: Volunteers gained 4 ± 1.3 kg and subsequently lost 3.2 ± 1.5 kg of body weight. TTST levels did not change (T1: 18.85 ± 6.2 nmol/L, T2: 19.3 ± 6.9 nmol/L, T3: 20.3 ± 7.6 nmol/L). However, SHBG decreased during weight gain (p=0.04) and increased during weight loss (p=0.0005) (T1: 22.8 ± 10.5 nmol/L, T2: 20.8 ± 11.1 nmol/L, T3: 25.58 ± 11.5 nmol/L). Furthermore, SHBG reduction during weight gain tended to correlate with decreases in FMD (p=0.06; R 2 =0.15). During weight loss, increases in SHBG correlate negatively with changes in FMD (p=0.008; R 2 =0.32). Conclusion : In normal weight healthy men, SHBG decreases with modest weight gain and increases with weight loss. TTST does not change. Changes in SHBG partly predict improvements in endothelial function with weight loss. Grant Funding Source : Supported by AHA 11SDG7260046, R21 DK81014, AHA 13POST16420009

Abstract Context Guidelines for the dosage of vitamin D supplementation vary widely globally. Objective To investigate the impact of 2 vitamin D doses, bracketed between the IOM recommended dietary allowance (RDA) and the upper tolerable limit, on vitamin D nutritional status in elderly individuals. Methods This post hoc analysis of data collected from a 12-month, double-blind, randomized control trial included 221 ambulatory participants (≥ 65 years) with a mean BMI of 30.2 kg/m2 and a mean baseline serum 25-hydroxyvitamin D [25(OH)D] level of 20.4 ± 7.4 ng/mL, who were recruited from 3 outpatient centers in Lebanon. All participants received 1000 mg of elemental calcium daily from calcium citrate plus the daily equivalent of either 600 IU or 3750 IU of vitamin D3. Results Mean 25(OH)D level at 12 months was 26.0 ng/mL with low dose and 36.0 ng/mL with high dose vitamin D3. The proportion of participants reaching a value ≥ 20 ng/mL was 86% in the low dose, and 99% in the high dose arms, with no gender differences. The increment of 25(OH)D per 100 IU/day was 1 ng/mL with the low dose, and 0.41 ng/mL with the high dose. Serum 25(OH)D levels at 1 year were highly variable in both treatment arms. Baseline 25(OH)D level and vitamin D dose—but not age, BMI, gender, or season—were significant predictors of serum 25(OH)D level post-intervention. Conclusion The IOM Recommended Dietary Allowance (RDA) of 600 IU/day does not bring 97.5% of ambulatory elderly individuals above the desirable threshold of 20 ng/mL. Country-specific RDAs are best derived taking into account the observed variability and predictors of achieved 25(OH)D levels.

Abstract Introduction: Immunoassay technology is subject to matrix interferences that can produce inaccurate results and incorrect conclusions when using samples not previously validated. While many commercially available research grade (RG) immunoassay kits are available, caution should be applied when using RG kits for thyroid health assessments, particularly on samples from pregnant individuals whose blood chemistry is unique to non-pregnant individuals. Question: Do RG immunoassay kits reliably provide precise and accurate measurements of thyroid health biomarkers in serum Standard Reference Materials (SRMs) from pregnant and non-pregnant donors? Methods: T4, T3, rT3, Tg and TSH measurements were conducted on SRMs 971, Hormones in Frozen Human Serum, and SRM 1949, Frozen Prenatal Serum, using RG immunoassay kits. When available, performance was assessed against validated FDA approved immunoassays or mass spectrometric (MS) methods. Results: RG kits were variable, inaccurate, or imprecise for four of the six biomarkers assessed. RG kit total thyroid hormone measurements overall performed comparably to MS methods, except rT3 measurements, which were twofold greater than mass spectrometric measurements (971M RG mean = 0.51 ng/mL, MS mean = 0.20 ng/mL, p < 0.0001; 971F RG = 0.48 ng/mL, MS = 0.18 ng/mL, p < 0.0001) and had CVs over 30 %. RG kit Tg measurements varied sometimes by as much as tenfold (971M means of 6.50 ng/mL up to 63.3 ng/mL, p < 0.0001; 971F means of 0.350 ng/mL up to 14.5 mg/mL, p < 0.0001). TSH values differed by RG kit manufacturer (971M means of 1.27 µIU/mL up to 1.82 µIU/mL, p < 0.0001; 971F means of 1.36 µIU/mL up to 2.27 µIU/mL, p < 0.0001) and by dilution scheme using the same manufacturer with one case indicating a diagnosis of hypothyroid versus normal TSH levels (1949 non-pregnant undiluted mean = 2325 pg/mL, half dilution mean = 1631 pg/mL, p < 0.0001). Conclusions: RG immunoassays are often used for research projects because they do not require expensive equipment and are simple to conducted. However, we demonstrate here that not all kits are accurate for all patient samples. By utilizing a matrix matched SRM with well-defined quantities of thyroid health biomarkers, one can assess method accuracy, making measurements from different methods comparable. Thereby, data can be harmonized to contribute reliable data on thyroid biomarkers to advance the field of thyroid health.

Abstract Background: Frailty is characterized by an increased vulnerability and a decline in physiological reserve. Frailty has been previously linked to cortisol concentrations and blunted diurnal cortisol secretion. Our objective was to determine the association of urine steroid metabolome and its diurnal variation with frailty and prefrailty in older adults. Methods: Cross-sectional study of community-dwelling adults ≥ 50 years. Participants with adrenal disorders, end-stage renal or liver disease, on exogenous steroids or drugs affecting steroid metabolism were excluded. All participants completed day and night separate urine collection. Frailty was assessed using a phenotype model (weight loss, exhaustion, grip strength, low physical activity, and slow walking pace). Participants were characterized as frail if they met at least three criteria, pre-frail if they fulfilled one or two criteria, and robust if no criteria were met. Urine samples were analyzed with the liquid-chromatography, high-resolution, accurate-mass mass spectrometry for 25 urine steroid metabolites. Results: Of 119 participants, 60 (50.4%) were women, without sex differences in age or education status. On frailty assessment, 5 (4.2%) participants were frail and 33 (27.7%) were prefrail, with equal sex distribution. Urine steroid metabolome analysis demonstrated 21/25 steroids were higher in men vs women. In an age adjusted model, presence of prefrailty or frailty was associated with a higher ratio of total cortisol metabolites/total androgen metabolites (TCM/TAM) in men (estimate 0.64, P-value= 0.0004), but not in women. In men, after adjusting for age, among cortisol metabolites, lower day to night ratio of 5α-Tetrahydrocortisol (estimate -0.36, P-value= 0.0419) and β-Cortol (estimate -0.35, P-value= 0.0238) were associated with frail or prefrail phenotype. After adjusting for age, higher ratio of TCM/TAM was associated lower gait speed in men (estimate -1.2, P-value= 0.046) and women (estimate -3.9, P-value= 0.012); and lower hand grip strength in men (estimate -0.04, P-value= 0.046) but not in women. Conclusion: We showed that a higher glucocorticoid to androgen ratio and a flattened circadian steroid variation were associated with presence of frail or prefrail phenotype in men. Further studies should examine the role of steroid metabolism and HPA axis impairment, and the associated sex differences, in the functional decline in aging population.

Abstract Background: Elevated urine cortisol (<1% of urinary steroid metabolome) was reported to predict future development of dementia. Our objective was to determine the association of urine steroid metabolome and its diurnal variation with cognitive function in men and women. Methods: Cross-sectional study of community-dwelling adults ≥ 50 years. Participants with adrenal disorders, end-stage renal or liver disease, on exogenous steroids or drugs affecting steroid metabolism were excluded. All participants completed day and night separate urine collection. A series of seven IPad-based tests using the National Institute of Health Toolbox Cognition Battery were administered to evaluate five key domains; performance was reported using fully corrected T-scores for age, sex, education, and race with a national normative mean of 50. T-scores were generated for the two summary measures: 1) fluid cognition (includes executive function, episodic memory, working memory, and processing speed), and 2) total composite (composite of fluid and language score). Urine samples were analyzed with the liquid-chromatography, high-resolution, accurate-mass mass spectrometry for 25 urine steroid metabolites. Results: Of 109 participants, 56 (51%) were women, and age and educational status were similar in men and women. On cognitive assessment, men and women had similar median composite cognition (T-score of 53 vs 54, p=0.74) and fluid cognition (T-score of 53 vs 51, p-value 0.96). Urine steroid metabolome analysis demonstrated 21/25 steroids were higher in men vs women. In both women and men, the ratio of total cortisol metabolites/total androgen metabolites (TCM/TAM) was associated with lower fluid cognition (women: ρ= -0.34, p=0.01, men: ρ= -0.43, p=0.001) and composite cognition (women: ρ= -0.27, p=0.04, men: ρ= -0.39, p=0.004). Higher ratio of day to night TCM were associated with a better fluid cognition in men (ρ= 0.35, p=0.01), but not in women (ρ= -0.11, p=0.41). Steroid ratios suggesting a relative enzymatic deficiency of 5α-Reductase type 2 was associated with lower fluid cognition in women (ρ= -0.29, p=0.03). In men, the fluid composite score was associated with a relative deficiency in 21-Hydroxylase (ρ= 0.42, p=0.002), 3β-Hydroxysteroid dehydrogenase (ρ= 0.43, p=0.001), and P450oxidoreductase (ρ= -0.35, p=0.01). Conclusion: We showed that a higher glucocorticoid to androgen ratio and a flattened circadian steroid variation were associated with lower global and fluid cognition score. Steroid ratios reflecting steroidogenesis enzymatic activity demonstrated sex differences in relation to cognition. Additional studies should examine whether the steroid fingerprint associated with lower cognition is predictive of a future dementia onset.

Significance Statement Expression of immediate early response gene X-1 ( Iex-1 ), also known as Ier-3, is increased in venous neointimal hyperplasia (VNH) and stenosis in hemodialysis arteriovenous fistulas (AVF). In a porcine model, 1α,25(OH) 2 D 3 , an inhibitor of Ier3, reduced VNH/stenosis formation. The 1α,25(OH) 2 D 3 released in the perivascular AVF space from poly(lactic-co-glycolic acid) nanoparticles embedded in a Pluronic F127 hydrogel (1,25 NP) reduced Ier3 gene and protein expression, MCP-1, CD68, HIF-1α, and VEGF-A immunostaining. Concomitantly, blood flow, lumen area, pulse wave velocity, and Young’s modulus increased, and neointima area, peak systolic velocity, and wall shear stress decreased in 1,25 NP–treated vessels. RNA sequencing analysis identified 242 genes involved in inflammatory and apoptotic pathways that had decreased expression in 1,25 NP vessels. Background Few therapies prevent venous neointimal hyperplasia (VNH) and venous stenosis (VS) formation in arteriovenous fistulas (AVF). Expression of the immediate early response gene X-1 ( Iex-1 ), also known as Ier3, is associated with VNH and stenosis in murine AVFs. The study aimed to determine if local release of Ier3 long-acting inhibitor 1α,25(OH) 2 D 3 from poly(lactic-co-glycolic acid) (PLGA) nanoparticles embedded in a thermosensitive Pluronic F127 hydrogel (1,25 NP) could affect VNH/VS formation in a large animal model. Methods Immediately after AVF creation in a porcine model of renal failure, 1,25 NP or vehicle control was injected into the adventitia space of AVF outflow veins. Scanning electron microscopy and dynamic light scattering characterized drug and control nanoparticles. Animals were sacrificed 3 and 28 days later for gene expression, immunohistologic, magnetic resonance imaging and angiography, and ultrasound analyses. Whole transcriptome RNA sequencing with differential gene expression analysis was performed on outflow veins of AVF. Results Encapsulation of 1α,25(OH) 2 D 3 in PLGA nanoparticles formed nanoparticles of uniform size that were similar to nanoparticles without 1α,25(OH) 2 D 3 . The 1,25 NP–treated AVFs exhibited lower VNH/VS, Ier3 gene expression, and IER-3, MCP-1, CD68, HIF-1α, and VEGF-A immunostaining, fibrosis, and proliferation. Blood flow and lumen area increased significantly, whereas peak systolic velocity and wall shear stress decreased. Treatment increased Young’s modulus and correlated with histologic assessment of fibrosis and with no evidence of vascular calcification. RNA sequencing analysis showed changes in the expression of genes associated with inflammatory, TGFβ1, and apoptotic pathways. Conclusions Local release of 1,25 NP improves AVF flow and hemodynamics, and reduces stenosis in association with reduction in inflammation, apoptosis, and fibrosis in a porcine model of arteriovenous fistula.

AbstractObjectiveDescribe the secretion and profile of adrenal steroids in patients with adrenal incidentalomas compared to control subjects.Design, Setting and ParticipantsA prospective study, 73 patients with adrenal incidentalomas, 21 bilateral and 52 unilateral and 34 matched controls in University Hospital.MethodsCollect fasting blood sample before and 60 min after ACTH test (250 µg IV). One week later, perform overnight 1 mg dexamethasone test. The following steroids were measured by liquid chromatography‐mass spectrometry (LC‐MS): pregnenolone, 17‐OH pregnenolone, 17‐OH progesterone, 11‐deoxycorticosterone, 11‐deoxyortisol, 21‐deoxycortisol, corticosterone, cortisol, androstenedione and aldosterone.ResultsMean baseline serum cortisol was higher in incidentalomas, bilateral 361 ± 124, (range 143–665) nmol/L,(p < .0001), unilateral 268 ± 89 3.2 (range 98–507) nmol/L (p < .019) compared to controls 207 ± 100 (range 72–502) nmol/L. ACTH stimulation showed significantly higher levels in bilateral and unilateral cases compared to controls. After dexamethasone, mean serum cortisol levels suppressed in bilaterals 89 ± 69 (range 30–3) nmol/L (p < .0001), 58 ± 52 (range 16–323) nmol/L in unilateral (p < .01) compared to 26 ± 9 (range 7–46) nmol/L in controls. Mean baseline serum corticosterone was higher in bilateral 9.3 ± 4.8 (range 2.4–18.4) nmol/L (p < .005) and unilateral 7.3 ± 5.7 (range 0.1–30.3) nmol/L (p < .01) compared to controls 4.2 ± 2.4 (range 1.1–10.2) nmol/L, after ACTH stimulation significantly increased to higher levels in bilateral (p < .0002) and unilateral cases (p < .044) compared to controls. After dexamethasone, mean levels were 2.5 ± 2.6 (range 0.5–12.5) nmol/L in bilateral (p < .0006), 1.5 ± 1.6 (range 0.3–9.3) nmol/L in unilateral (p < .09) and 0.75 ± 0.46 (range 0.1–2.1) nmol/L in controls. Mean baseline serum 11‐deoxycorticosterone (DOC) was higher in bilaterals 0.32 ± 0.23 (range 0.08–1.1) nmol/L (p < .03) compared to controls 0.15 ± 0.21 (range 0.08–1.1) nmol/L. ACTH stimulation increased levels to 3.27 ± 1.72 (range 0.5–7.4) nmol/L in bilateral cases compared to controls 1.369 ± 1.53 (range 0.1–7.1) nmol/L (p < .0001). Dexamethasone decreased levels to baseline (p ns). There were significant differences in serum 21‐deoxycortisol (p < .0002) and serum pregnenolone (p < .004) only after ACTH stimulation.ConclusionsThere is increased activity in several steroid biosynthesis pathways and higher steroid levels in bilateral compared to unilateral cases and evidence of hypercortisolism in 30% unilateral and 62% of bilateral incidentalomas.

Abstract Context The normal cortisol response 30 or 60 minutes after cosyntropin (ACTH[1–24]) is considered to be ≥18 μg/dL (500 nmol/L). This threshold is based on older serum cortisol assays. Specific monoclonal antibody immunoassays or LC-MS/MS may have lower thresholds for a normal response. Objective To calculate serum cortisol cutoff values for adrenocorticotropic hormone (ACTH) stimulation testing with newer specific cortisol assays. Methods Retrospective analysis of ACTH stimulation tests performed in ambulatory and hospitalized patients suspected of adrenal insufficiency (AI). Serum samples were assayed for cortisol in parallel using Elecsys I and Elecsys II immunoassays, and when volume was available, by Access immunoassay and LC-MS/MS. Results A total of 110 patients were evaluated. Using 18 μg/dL as the cortisol cutoff after ACTH stimulation, 14.5%, 29%, 22.4%, and 32% of patients had a biochemical diagnosis of AI using the Elecsys I, Elecsys II, Access, and LC-MS/MS assays, respectively. Deming regressions of serum cortisol were used to calculate new cortisol cutoffs based on the Elecsys I cutoff of 18 μg/dL. For 30-minute values, new cutoffs were 14.6 μg/dL for Elecsys II, 14.8 μg/dL for Access, and 14.5 μg/dL for LC-MS/MS. Baseline cortisol <2 μg/dL was predictive of subnormal stimulated cortisol values. Conclusion To reduce false positive ACTH stimulation testing, we recommend a new serum cortisol cutoff of 14 to 15 μg/dL depending on the assay used (instead of the historical value of 18 μg/dL with older polyclonal antibody assays). Clinicians should be aware of the new cutoffs for the assays available to them when evaluating patients for AI.
