MRS in Mild Cognitive Impairment: Early Differentiation of Dementia with Lewy Bodies and Alzheimer's Disease

Journal of Neuroimaging | July 16, 2014 | Volume: 25 | Issue: 2 | Pages: 269-274

ABSTRACTBACKGROUND AND PURPOSEMild cognitive impairment (MCI) precedes both Alzheimer's disease (AD) dementia and with Lewy bodies (DLB). We investigated proton magnetic resonance spectroscopy (MRS) characteristics of MCI patients who progressed to DLB compared to those who progressed to AD dementia or remained stable.METHODSConsecutive MCI patients who underwent single voxel MRS at baseline and progressed to DLB (n = 10) were identified during a median follow‐up period of 18 months. From the same cohort, we identified age‐ and sex‐matched MCI patients who progressed to AD dementia (n = 27) or remained stable (n = 20) during a similar follow‐up period. This study was approved by the Institutional Review Board and informed consent was from every subject.RESULTSMCI patients who progressed to AD dementia were characterized by lower N‐acetylaspartate (NAA)/Cr ratio in the posterior cingulate voxel compared to those who progressed to DLB (P = .001). Decreased NAA/Cr in the posterior cingulate voxel differentiated MCI patients who progressed to DLB from those who progressed to AD with an area under the receiver operating characteristic curve of .85 (P < .001) on logistic regression analysis.CONCLUSIONSMRS may be useful in differentiating MCI patients with prodromal AD dementia from those with prodromal DLB for early disease‐specific interventions.
Operationalizing protocol differences for EADC‐ADNI manual hippocampal segmentation

AbstractBackgroundHippocampal volumetry on magnetic resonance imaging is recognized as an Alzheimer's disease (AD) biomarker, and manual segmentation is the gold standard for measurement. However, a standard procedure is lacking. We operationalize and quantitate landmark differences to help a Delphi panel converge on a set of landmarks.MethodsOne hundred percent of anatomic landmark variability across 12 different protocols for manual segmentation was reduced into four segmentation units (the minimum hippocampus, the alveus/fimbria, the tail, and the subiculum), which were segmented on magnetic resonance images by expert raters to estimate reliability and AD‐related atrophy.ResultsIntra‐ and interrater reliability were more than 0.96 and 0.92, respectively, except for the alveus/fimbria, which were 0.86 and 0.77, respectively. Of all AD‐related atrophy, the minimum hippocampus contributed to 67%; tail, 24%; alveus/fimbria, 4%; and the subiculum, 5%.ConclusionsAnatomic landmark variability in available protocols can be reduced to four discrete and measurable segmentation units. Their quantitative assessment will help a Delphi panel to define a set of landmarks for a harmonized protocol.

Cardiovascular risk factors, cortisol, and amyloid‐β deposition in Alzheimer's Disease Neuroimaging Initiative

AbstractBackgroundThere is epidemiological evidence that cardiovascular risk factors (CVRF) also are risk factors for Alzheimer's disease, but there is limited information on this from neuropathological studies, and even less from in vivo studies. Therefore, we examined the relationship between CVRF and amyloid‐β (Aβ) brain burden measured by Pittsburgh Compound B‐positron emission tomography (PiB‐PET) studies in the Alzheimer's Disease Neuroimaging Initiative.MethodsNinety‐nine subjects from the Alzheimer's Disease Neuroimaging Initiative cohort who had a PiB‐PET study measure, apolipoprotein E genotyping data, and information available on CVRF (body mass index [BMI], systolic blood pressure, diastolic blood pressure [DBP], and cholesterol and fasting glucose test results) were included. Eighty‐one subjects also had plasma cortisol, C‐reactive protein, and superoxide dismutase 1 measurements. Stepwise regression models were used to assess the relation between the CVRF and the composite PiB‐PET score.ResultsThe first model included the following as baseline variables: age, clinical diagnosis, number of apolipoprotein ε4 alleles, BMI (P = .023), and DBP (P = .012). BMI showed an inverse relation with PiB‐PET score, and DBP had a positive relation with PiB‐PET score. In the second adjusted model, cortisol plasma levels were also associated with PiB‐PET score (P = .004). Systolic blood pressure, cholesterol, or impaired fasting glucose were not found to be associated with PiB‐PET values.ConclusionIn this cross‐sectional study, we found an association between Aβ brain burden measured in vivo and DBP and cortisol, indicating a possible link between these CVRF and Aβ burden measured by PiB‐PET. These findings highlight the utility of biomarkers to explore potential pathways linking diverse Alzheimer's disease risk factors.
