This study assessed the patterns of nonhospice health care utilization among 207 deceased cancer patients and focused on outcomes after cancer treatment was stopped. A total of 117 (57%) were enrolled in hospice. The mean cumulative number of emergency department visits, hospitalizations, or other noncancer clinic visits (standard deviation) among those enrolled and not enrolled in hospice was 1.8 (± 1.8) and 3.11 (± 3.0), respectively ( P < .0001). Among hospice enrollees, the mean cumulative visits (standard deviation) was 1.29 (± 1.7) and 0.5 (± 1.0) before and after enrollment, respectively ( P < .0001). For patients who eventually enrolled, the rates of nonhospice health care visits (visits per week) yielded a trend to suggest a decline ( P = .054). Hospice was associated with a drop in nonhospice-related health care utilization, thus suggesting it provides timely medical interventions and favorable continuity of care.
Adjuvant Chemotherapy and Targeted Therapy in Elderly Non-Small-Cell Lung Cancer Patients
Aging Health | June 25, 2012 | Volume: 8 | Issue: 3 | Pages: 309-316
Extraosseous Ewing's sarcoma (EES) involving the central nervous system is rare, but can be diagnosed and distinguished from other primitive neuroectodermal tumors (PNET) by identification of the chromosomal translocation (11;22)(q24;q12). We report EES arising from the spinal intradural extramedullary space, based on imaging, histopathological, and molecular data in two men, ages 50 and 60 years old and a review of the literature using PubMed (1970–2009). Reverse transcriptase polymerase chain reaction (RT-PCR) identified the fusion product FL1-EWS. Multimodal therapy, including radiation and alternating chemotherapy including vincristine, cyclophosphamide, doxorubicin and ifosfamide and etoposide led to local tumor control and an initial, favorable therapeutic response. No systemic involvement was seen from the time of diagnosis to the time of last follow-up (26 months) or death (4 years). This report confirms that EES is not confined to the earliest decades of life, and like its rare occurrence as an extra-axial meningeal based mass intracranially, can occasionally present as an intradural mass in the spinal canal without evidence of systemic tumor. Gross total resection followed by multimodal therapy may provide for extended progression free and overall survival.
Abstract 1807: Phase I trial of the combined targeting of the Hedgehog (GDC-0449) and EGFR pathways (erlotinib) in pancreatic cancer
Cancer Research | February 25, 2011 | Volume: 70 | Issue: 8_Supplement | Pages: 1807-1807
Abstract We have demonstrated that crosstalk between the Hedgehog-GLI-GLI and EGFR signaling pathway exists and contribute to pancreatic tumorigenesis. This interaction can be abrogated by targeting both pathways simultaneously and results in a synergistic antitumor effect in vitro and in vivo in pancreatic tumor models. Based on these results we have undertaken a phase I trial of GDC-0449 (an orally administered Hedgehog pathway inhibitor) and erlotinib (EGFR tyrosine kinase inhibitor to: 1) determine the maximally tolerated dose (MTD) of GDC-0449 combined with erlotinib in unresectable solid tumors; 2) describe the adverse events associated with this treatment combination; 3) describe the responses of this treatment combination; and 4) to assess the effect of the erlotinib and GDC-0449 combination on selected biomarkers in circulating tumor cells (CTCs), tumor biopsies, and FDG-PET in a cohort of patients with metastatic pancreatic cancer patients treated at the MTD. During the dose escalation portion of the trial, eligible patients receive GDC-0449 150 mg p.o. daily and erlotinib 50, 75, 100, or 150 mg daily p.o. at dose levels 1, 2, 3, and 4 respectively. Eligibility criteria include: histologic proof of solid tumors that are unresectable, not amenable to any other standard therapies, or patient refused standard therapy,; ANC ≥> 1500/μL; PLT ≥> 100,000/ μL; Total bilirubin ≤< upper limit of normal (ULN); AST ≤< 3 x ULN; Creatinine ≤< 1.5 x ULN; Hemoglobin ≥> 9.0 g/dL; INR within normal limits (Cohort II [MTD Cohort] only); ECOG performance status (PS) 0, 1 or ≤ 2. For the MTD cohort, only patients with metastatic adenocarcinoma of the pancreas without prior systemic therapy for metastatic disease and tumor amenable to biopsies are eligible. Toxicity assessment is performed at each 28-day treatment cycles and response assessment every second treatment 28 day cycle. 12 patients have been entered-pancreas (3), colorectal (3), neuroendocrine (2), scc anus (2), gallbladder (1), thymoma (1). No dose-limiting toxicity (DLT) was reported at dose level 4. At the MTD, up to 20 patients with previously untreated metastatic pancreatic cancer will be entered to assess the biologic effect of GDC-0449+erlotinib. Pre- and post- treatment biopsies of metastases and FDG-PET will be performed. Weekly collection of CTCs will be undertaken and immunofluorescence microscopy of total & phosphoMAPK, EGFR, AKT, PATCHED1, GLI1, BCL-2, and quantitative PCR (Q-PCR) of PATCHED1, GLI1, and its target genes (BCL-2, BFL-1/A1, 4-1BB) will be performed. In addtion, total & phosphoMAPK, EGFR, and AKT, as well as PATCHED1, GLI1, BCL-2 by immunohistochemistry and Q-PCR of PATCHED1, GLI1, BCL-2, BFL-1/A1, 4-1BB will be analyzed in tumor biopsies. Does Sunscreen Prevent Epidermal Growth Factor Receptor (EGFR) Inhibitor–Induced Rash? Results of a Placebo-Controlled Trial from the North Central Cancer Treatment Group (N05C4)
The Oncologist | August 27, 2010 | Volume: 15 | Issue: 9 | Pages: 1016-1022
Abstract Purpose. Rash occurs in >50% of patients prescribed epidermal growth factor receptor (EGFR) inhibitors. This study was undertaken to determine whether sunscreen prevents or mitigates these rashes. Methods. This placebo-controlled, double-blinded trial enrolled rash-free patients starting an EGFR inhibitor. Patients were randomly assigned to sunscreen with a sun protection factor of 60 applied twice a day for 28 days versus placebo. They were then monitored for rash and quality of life (Skindex-16) during the 4-week intervention and for an additional 4 weeks. Results. Fifty-four patients received sunscreen, and 56 received placebo; the arms were balanced at baseline. During the 4-week intervention, physician-reported rash occurred in 38 (78%) and 39 (80%) sunscreen-treated and placebo-exposed patients, respectively (p = 1.00); no significant differences in rash rates emerged over the additional 4 weeks. There were no significant differences in rash severity, and patient-reported outcomes of rash yielded similar conclusions. Adjustment for sun intensity by geographical zone, season, and use of photosensitivity medications did not yield a significant difference in rash across study arms (p = .20). Quality of life scores declined but remained comparable between arms. Conclusions. Sunscreen, as prescribed in this trial, did not prevent or attenuate EGFR inhibitor–induced rash.

Without adequate safety measures, oral chemotherapy can lead to undetected dosing errors. The Mayo Clinic launched a project to ensure that all capecitabine and temozolomide prescriptions receive an independent double check.
